The goal of the MIFD Lyme Disease Prevention Committee is to encourage universal tick control by providing a program both affordably priced and ecologically responsible.

Please take a few minutes to print out this email. Consider the options available, complete the form and answer the questions. Due to the time needed to organize this program, please respond by March 1, 2015. Please return the form on the bottom of this email to the following address:

For the summer of 2015, the MIFD Tick Control Program includes a choice of two selected vendors who offer us discounted rates: SeaScape Inc., who treats with a chemical insecticide; and BioTech Pest Controls, who treats with an organic-based tick control. If you participate in the MIFD program, the cost of your chosen tick control method will be billed to you as a supplemental "tick tax" due March 31, 2015.

Additionally, we ask that you answer questions concerning your household in relation to tick control and disease occurrence. This information will help us track the program's effectiveness and will be used in the aggregate for the DEP scientific study.

I hereby give SeaScape Inc. permission to apply three applications of a Bifenthrin based tick control insecticide to the listed properties for the 2015 tick season for a total cost of $190/property. The insecticide will be applied to the perimeter of each property excluding wetlands. I understand $190/property will be billed to me as a supplemental "tick tax" due March 31, 2015.

I hereby give BioTech Pest Control permission to apply two applications of an organic based insecticide, Ecotrol EC and Ecopco G (active ingredients: Rosemary and Peppermint Oil), to the listed properties for the 2015 tick season for a total cost of $225/property. I understand $225/property will be billed to me as a supplemental "tick tax" due March 31, 2015.
