The Affordable Care Act (commonly known as Obamacare) encourages the formation of "medical homes" and "accountable care organizations" (ACOs), in the belief that they will improve health care quality and slow the growth of health care spending in America. By organizing health care teams, technology and knowledge around patient needs, Accountable Care is supposed to help the new system realize its full potential.

Doctors and health care providers can establish ACOs in order to work together to provide higher-quality care to their patients. Since passage of the Affordable Care Act, more than 250 Accountable Care Organizations have been established. Beneficiaries using ACOs always have the freedom to choose doctors inside or outside of the ACO. ACOs share with Medicare any savings generated from lowering the growth in health care costs, while meeting standards for quality of care.

The Congressional intent behind expanding Medicaid eligibility so significantly was to help address the growing ranks of the uninsured in America. Currently it is estimated that 48 million Americans are without medical insurance. While the exact number of Americans who would be covered as a result of the PPACA and such provisions as the Medicaid eligibility expansion is debatable, there is some consensus that 32 million Americans will gain health coverage through reform efforts signed into law.

Medical debt is included in the FDCPA because it is a patient's responsibility to take care of bills that result from treatment. Providers, beware of your collection agency practices. Given that debt collectors are protected by the FDCPA, the patient can still initiate legal action against any collector that violates the guidelines of the Act based on their methods.

Yesterday I was in the doctor's waiting room and I heard a ninety-six-year-old man pleading with the doctor for a lower sex drive.
'Surely you're imagining things,' said the doctor. 'You're ninety-six years old. Isn't all the feeling for sex just in your head?'
'Yes,' replied the elderly man, 'that's why I want you to lower my sex drive to the place where it might do more good.'

A fellow walked into a doctor's office and the receptionist asked him what he had.
He said, "Shingles."
So she took down his name, address, medical insurance number and told him to have a seat.

A few minutes later a nurse's aid came out and asked him what he had.
He said, "Shingles."
So she took down his height, weight, a complete medical history and told him to wait in the examining room.

Ten minutes later a nurse came in and asked him what he had.
He said, "Shingles."
So she gave him a blood test, a blood pressure test, an electrocardiogram, told him to take off all his clothes and wait for the doctor.

Fifteen minutes later the doctor came in and asked him what he had.
He said, "Shingles."
The doctor said, "Where?"
He said, "Outside in the truck. Where do you want them?"

An acquaintance of mine who is a physician told this story about her then four-year-old daughter. On the way to preschool, the doctor had left her stethoscope on the car seat, and her little girl picked it up and began playing with it. Be still, my heart, thought my friend, my daughter  wants to follow in my footsteps! 

Then the child spoke into the instrument: "Welcome to McDonald's. May I take your order?"

An elderly woman went into the doctor's office. When the doctor asked why she was there, she replied, "I'd like to have some birth control pills."

Taken aback, the doctor thought for a minute and then said, "Excuse me, Mrs. Smith, but you're 75 years old. What possible use could you have for birth control pills?"
The woman responded, "They help me sleep better."

The doctor thought some more and continued, "How in the world do birth control pills help you to sleep?"

The woman said, "I put them in my granddaughter's orange juice and I sleep better at night."
