What are Outreach Services?

"Outreach services" can be broadly defined as the provision of a set of services that identify appropriate candidates for treatment, and provide referrals and placement, intervention and other information that supports and enables access to treatment and recovery.

 

The definition of outreach services is quite broad, and often encompasses many forms of mental and physical health. We specifically provide outreach related to addiction and mental health, providing information related to intervention, detox from drugs and alcohol, adolescent treatment, eating disorders, trauma disorders, drug and alcohol addiction, and patients with a dual diagnosis of addiction and a co-occurring disorder such as depression and anxiety.

 

In addition to assisting with assessments as to what scope of treatment is needed, we specialize in providing referrals to appropriate specialists or facilities, and can help with insurance and financing questions. We work with patients, families and professionals in the field, and our services are free to the public and confidential.

Pat Kelly 
Certified Addiction Treatment Counselor and Board Registered Interventionist


Pat Kelly is a member of the Association of Intervention Specialists. She  has been helping people with addictions in Calif. for over 25 years.  She has been married 43 years, has 4 children and 3 grandchildren. 

 

She worked in Psychiatric Hospitals for 10 years and was Director of Intake and Assessment. She put together an intake training packet several years ago and trains facilities on answering  phones.   If anyone wants a copy she will be happy to give them one. She has worked at different treatment facilities, was an EAP Consultant, mentors interventionists and is currently a consultant for facilities with their Utilization Review for insurance companies. She does interventions on all ages and all addictions and would like to start doing them on obese people.

 

She is also a member of the Emergency Response Intervention Team in Orange County. They respond on an emergency basis and help the addicted person get the proper placement immediately.

 

In October of 2007 Pat was diagnosed with colon cancer. She did chemo and radiation and had surgery at City of Hope in February of 2008. She is currently 5 years cancer free and lives everyday to the fullest.

 

In June of 2012 she was Honored with the Sierra Tucson Gratitude Lifetime Achievement Award in Beverly Hills for her support and help in turning peoples lives around in the addiction field.

Sherry Gaba is a Psychotherapist and Life Coach specializing in individual, couples, family, and group psychotherapy and maintains a private practice in Westlake Village, CA located near Malibu, Westlake, Calabasas, Woodland Hills, West Hills, Thousand Oaks, Newbury Park, Simi Valley, and Moorpark, CA.

Never Ending Need for Documentation
As you all know insurance companies are implementing procedures to delay payments. One of the variables; NO Authorization for PHP and IOP.
This initially feel like coverage is a guarantee. It may or may not be! What several of the insurance companies are doing following sending the claim; is asking for the chart to review appropriateness of services.
The documentation you send must meet all criteria for all levels of care that were utilized by your facility. Starting with the Level of Care Assessment Tool (LOCAT) a common  guideline to help evaluate the patient's symptoms utilizing the health dimensions to assist with appropriate placement:
    1. Acute Dangerousness: suicidal. homicidal and irritability/Aggression/Mania Include level of risk in terms of detox needs, medical/behavioral
    2. Functional Impairment: Determine the severity of impairment in function ie: social isolation, nutritional, sleep, work and home environment.
    3. Mental Status and Co-morbid Factors: patient's stressors, including appearance, thought process, delusions, or hallucinations, behavioral/neurological and orientation and co-occurring substance abuse.
    4. Psychosocial factors assess outside influences that may be affecting patient, ie: family, housing, school, employment and support systems.
    5. Additional other modifiers examines appropriate level of care needed, which may include previous treatment,personal resources:
