Over the last year I have seen a marked increase in studies involving probiotics. This last issue has six in it! The year before that it was Vitamin D that turned up frequently.

Effect of Withania somnifera (Ashwagandha) on the Development of Chemotherapy-Induced Fatigue and Quality of Life in Breast Cancer Patients.

 

This open-label prospective trial on 100 patients with breast cancer in all stages undergoing either a combination of chemotherapy with oral Withania somnifera (2g every 8 hours throughout the course of chemotherapy) or chemotherapy alone. The majority (77%) of patients had stage II and III disease. The withania  group statistically significant higher estimated marginal means of fatigue score compared with the control group. There was was also statistically significant in the symptom scales of the EORTC QLQ-C30  in 7 of the 18 symptoms in the withania group.

Safety and Efficacy Profile of Echinacea purpurea to Prevent Common Cold Episodes

 

 Seven hundred and fifty-five healthy subjects were allocated to receive either an alcohol extract from freshly harvested E. purpurea* (95% herba and 5% root) or placebo. Participants swallowed 3 × 0.9mL per day for illness prevention. This corresponded to 2400mg of extract per day. During acute stages of colds, the participants were instructed to increase the dose to 5 × 0.9mL per day; this totaled a daily dose of 4000mg of extract. Each single dose was diluted in water and retained in the mouth for 10s. A total of 293 adverse events occurred with Echinacea and 306 with placebo treatment. Echinacea reduced the total number of cold episodes, cumulated episode days within the group, and pain-killer medicated episodes. Echinacea inhibited virally confirmed colds and especially prevented enveloped virus infections. Echinacea showed maximal effects on recurrent infections, and preventive effects increased with therapy compliance and adherence to the protocol. 

 

*Echinacea product was the commercially available Echinaforce drops produced by A. Vogel Bioforce AG, Switzerland. Echinaforce was prepared by alcoholic (57.3% m/m) extraction from freshly harvested E. purpurea with a combination of 95% herba (DER = 1:12) and 5% roots (DER = 1:11).

Andrographis Paniculata Extract (HMPL-004) for Active Ulcerative Colitis.

Two hundred and twenty four adults with mild-to-moderate ulcerative colitis were randomly assigned to Andrographis paniculata  extract (HMPL-004) 1,200mg or 1,800mg daily or placebo for 8 weeks. Patients receiving A. paniculata 1,200mg and 1,800mg daily, respectively had a clinical response at week 8 in 45 and 60% compared with 40% in the placebo group. Thirty-four and 38% of patients receiving A. paniculata 1,200mg and 1,800mg daily, respectively, were in clinical remission at week 8 compared to 25% in the placebo group.

Efficacy and Safety of a Combination Herbal Medicinal Product containing Tropaeoli majoris herba and Armoraciae rusticanae radix for the Prophylactic Treatment of Patients with Respiratory Tract Diseases

Three hundred and fifty one subjects of both sexes, (18 to 75 years) were randomly allocated to one of the 3 groups.  Group 1 received the herbal tablets 3x2 per day, group 2 the herbal tablets 2x2 and placebo tablets 2x1 per day and group 3 received placebo tablets 3x2 per day and observed for the incidence of new occurring infections of the respiratory tract . The herbal tablet contained  horseradish root (Armoraciae rusticanae radix) and nasturtium (Tropaeoli majors herb). In the Intention to Treat (ITT) collective excluding early infections ≤ day 7 (n= 344) the infection rates were 13.3% for Group 1 (higher dose), 18.4% for group 2 (lower dose), and 25.6% for placebo . For the Per Protocol (PP) collective infection rates were: group 1 12.7%, group 2 18.6% and placebo 24.3%. Secondary parameters of infections (infection diagnosis, intensity, duration) showed no relevant differences between the treatment groups.

Efficacy and Safety of a Combination Herbal Medicinal Product containing Tropaeoli majoris herba and Armoraciae rusticanae radix for the prophylactic treatment of patients with respiratory tract diseases: A randomised, prospective, double-blind, placebo-controlled phase III trial. 

 

To Evaluate the Efficacy and Safety of Safoof Habis in Kasrate Tams (menorrhagia) with Validated Menstrual Pictogram.

Fifty-two patients diagnosed with menorrhagia  randomly assigned Safoof Habis (Ingredient are Silicate of alumina and Iron oxide, Hydrated magnesium silicate and Vateria indica L.) 5 g twice daily or tranexamic acid 500 mg, tid administered from day 1 to day 5 of menstrual cycle and Qurs Kushta Faulad, one tablet twice daily was administered in both the groups for three consecutive months. There was a  significant reduction in menstrual blood loss in both the groups.  The comparison between test and control group showed that both groups were equally effective There was improvement in hemoglobin concentration in the test group  and dysmenorrhea was reduced in both groups.

Chamomile (Matricaria recutita) May Provide Antidepressant Activity in Anxious, Depressed Humans

 

Fifty-seven participants in the 2009 trial, 19 had anxiety with comorbid depression; 16 had anxiety with a past history of depression; and 22 had anxiety with no current or past depression received 220-mg capsules containing either pharmaceutical-grade chamomile extract standardized to a content of 1.2% apigenin or a placebo. There was a significantly greater reduction over time in total Hamilton Depression Rating (HAM-D) scores for chamomile vs placebo. There was also a  clinically meaningful but nonsignificant trend for a greater reduction in total HAM-D scores for chamomile vs placebo in participants with current comorbid depression. There was a significantly greater reduction over time observed for chamomile vs placebo in all participants and a clinically meaningful but nonsignificant trend for a greater reduction over time for chamomile vs placebo in participants without current or past depression.

Pelargonium sidoides Root Extract EPs(®) 7630  for Prevention of Asthma Attacks During Viral Infections in Children

 

Sixty-one asthmatic children with upper respiratory tract infection were randomized to  Pelargonium sidoides (PS) and supportive treatment  or supportive treatment only for 5 days. The PS solution was dosed 1-5 years 10gtt. tid, 6-12 age 20 gtt. tid and 12 and above 30 get. tid. No strength was given for the extract. Paracetamol (antipiretic) was use as a supportive treatment. Fever, sleep and muscle aches were similar in both groups. There was a significant difference in nasal symptoms and cough frequency in the PS group compared to supportive treatment alone. The PS group had a significant reduction in asthma attack frequency.

St. John's Wort, Valerian root and Passionflower for Nervous Agitation in Children

 

This  prospective observational study looked at to what extent a fixed plant extract combination was able to support children suffering from nervous agitation due to agitated depression (115 children between 6 and 12 years). The plant extract contained St. John's Wort, Valerian root and Passionflower.  Assessments of the parents showed a distinct improvement in children who had attention problems, showed social withdrawal, and/or were anxious/depressive. Based on the physicians' assessment, 81.6-93.9 % of the affected children had no or just mild symptoms at the end of observation concerning nine of thirteen evaluated symptoms such as depression, school/examination anxieties, further anxieties, sleeping problems, and different physical problems. During the observation period, 16 children (13.9  %)received additional medications related to the inclusiondiagnosis, mainly hypnotics/sedatives (64  %) and psychopharmaceuticals/psychoanaleptics (12  %). Eightyfour percentages of the respective drugs were used aslong-term medication. Up to three nonmedical therapieswere reported for 61 children (53  %) such as psychological/psychotherapeutic care (46.6 %) and progressiverelaxation (19.2 %).

Dose-dependent Efficacy of the Vitex agnus castus Extract Ze 440 in Patients Suffering from Premenstrual Syndrome.

 

One-hundred and sixty-two women with PMS ( 18-45 years) were randomly assigned to Vitex agnus castus L. Ze 440 (8, 20 and 30mg) or placebo over three menstrual cycles. Improvement in the total symptom score (TSS) in the 20mg group was significantly higher than in the placebo and 8mg treatment group. The higher dose of 30mg, on the other hand, did not significantly decrease symptom severity compared to the 20mg treatment. Corresponding results were observed with the single PMS symptom scores (irritability, mood alteration, anger, headache, bloating and breast fullness).

Effect of Ground Cinnamon on Postprandial Blood Glucose Concentration in Normal-weight and Obese Adults.

 

Thirty (18-30 years) normal-weight and obese subjects (BMI's between 18.5 and 24.9 and 15 with BMIs of 30.0 or more) were given 50 g available carbohydrate in instant farina cereal, served plain or with 6 g ground cinnamon in this cross-over trial. The addition of cinnamon to the cereal significantly reduced 120-minute glucose area under the curve (AUC) and blood glucose at 15 (109 mg/dL vs. 99 mg/dL), 30, 45, and 60 (139mg/dL vs. 120 mg/dL) minutes. At 120 minutes, blood glucose was significantly higher with cinnamon consumption 109 vs 91 mg/dL.

Effect of Mindfulness Training on Asthma Quality of Life and Lung Function

 

This 8-week study evaluated mindfulness-based stress reduction (MBSR)  in adults with mild, moderate or severe persistent asthma. At 12 months MBSR resulted in clinically significant improvements from baseline in quality of life (differential change in Asthma Quality of Life Questionnaire score) for MBSR vs control  but not in lung function. MBSR also resulted in clinically significant improvements in perceived stress.  MBSR produced lasting and clinically significant improvements in asthma-related quality of life and stress in patients with persistent asthma, without improvements in lung function.

Effect of mindfulness training on asthma quality of life and lung function: a randomised controlled trial.  

 

Effects of Tai Chi exercise on Glucose Control, Neuropathy Scores, Balance, and Quality of Life in Patients with Type 2 Diabetes and Neuropathy.

 

Fifty-nine diabetics with neuropathy (mean age 64), diagnosed with diabetes more than 12 years were randomly assigned to Tai Chi or no intervention for 12 weeks.  The Tai Chi group participated in 1 a hour of Tai Chi session, twice a week. The Tai chi gourd had significantly better total symptoms scores,  glucose control, balance, and quality of life compared to the control group.

Manipulating Antioxidant Intake in Asthma

 

One hundred and thirty seven asthmatic adults were assigned to a high antioxidant diet ((5 servings of vegetables and 2 servings of fruit daily) or a low antioxidant diet (≤2 servings of vegetables and 1 serving of fruit daily) for 14 days and then commenced a parallel, randomized, controlled supplementation trial. Subjects who consumed the high-antioxidant diet received placebo. Subjects who consumed the low-antioxidant diet received placebo or tomato extract (45 mg lycopene/d) until week 14 or until an exacerbation occurred. After day 14 the low-antioxidant diet group had a lower percentage predicted forced expiratory volume in 1 s and percentage predicted forced vital capacity than did those consuming the high-antioxidant diet. The  low-antioxidant diet group had increased plasma C-reactive protein.  At the end of the trial, time to exacerbation was greater in the high-antioxidant than in the low-antioxidant diet group, and the low-antioxidant diet group was 2.26 times as likely to exacerbate. Of the subjects in the low-antioxidant diet group, no difference was observed between the groups that consumed the tomato extract and those who consumed placebo. Improvements were evident only after increased fruit and vegetable intake, which suggests that whole-food interventions are most effective.

Melatonin as Adjuvant Cancer Care With and Without Chemotherapy

 

Twenty-one clinical trials were  reviewed  for the effects of melatonin in conjunction with chemotherapy, radiotherapy, supportive care, and palliative care on 1-year survival, complete response, partial response, stable disease, and chemotherapy-associated toxicities. All trials  randomized patients to treatment, including melatonin or a similar control group without melatonin.  In trials combining melatonin with chemotherapy, adjuvant melatonin decreased 1-year mortality and improved outcomes of complete response, partial response, and stable disease. Melatonin also significantly reduced asthenia, leucopenia, nausea and vomiting, hypotension, and thrombocytopenia.

Cherry Consumption and the Risk of Recurrent Gout Attacks.

 

Six hundred and thirty three individuals with gout were followed for 1 year and asked to record the onset date of the gout attack, symptoms and signs, medications (including anti-gout medications), and potential risk factors (including daily intake of cherries and cherry extract) during the 2-day period prior to the gout attack. Cherry intake over a 2-day period was associated with a 35% lower risk of gout attacks compared to no intake. In those individuals consuming cherries 35 % ate them fresh, 2% ingested them as extract and 5% consumed cherry fruit and extract. A cherry serving was one half cup or 10-12 fruit. The gout flare risk continued to decease with increasing cherry consumption up to 3 servings over 3 days but more than that did not confer additional benefit. When cherry intake was combined with allopurinol use, the risk of gout attacks was 75% lower than periods without either exposure

Lactobacillus Plantarum CJLP133 for the Treatment of Atopic Dermatitis.

 

This randomized placebo controlled trial included 118 chiclren (12 months - 13 years) with atoptic dermatitis. The chiclred were given Lactobacillus plantarum CJLP133 at a dosage of 0.5 × 10(10)  colony-forming units or placebo twice a day for 12 weeks.  The mean change in the SCORAD score from weeks 2-14 was 9.1 in theprobiotic group and 1.8 in the control group. There were no statistical differences between the groups in total use of topical steroids. The probiotic group had a significantly lower total eosinophil count  at the end of the intervention compared to baseline measurements. Logarithmic IFN-γ and IL-4 were also significantly decreased in the probiotic group.

Oral Magnesium Supplementation in Children with Cystic Fibrosis Improves Clinical and Functional Variables

 

Forty-four cystic fibrosis (CF) patients (aged 7-19 years; 20 males) were randomly assigned to receive magnesium 300 mg/day or placebo for 8 weeks with a 4 week washout period between trials. All patients were undergoing conventional treatment of CF. oth maximal inspiratory pressure (MIP) and maximal expiratory pressure (MEP) significantly improved only after magnesium administration. A change in MIP of 11% was predicted after magnesium compared with 0.5% predicted after placebo; a change in MEP of 11.9%  was predicted after magnesium compared with 0.8% predicted after placebo.

Oral magnesium supplementation in children with cystic fibrosis improves clinical and functional variables: a double-blind, randomized, placebo-controlled crossover trial. 

 

Advantages of the Association of Resveratrol with Oral Contraceptives for Management of Endometriosis-related Pain.

 

Twelve patients who failed to obtain relief from endometriosis-related pain during use of an oral contraceptive containing drospirenone + ethinylestradiol were given 30 mg of resveratrol in addition to the contraceptive regimen for 2 months. At the end of 2 months 82% of the patients reported complete resolution of dysmenorrhea and pelvic pain. In a separate experiment, aromatase and cyclo-oxygenase-2 expression were investigated in the endometrial tissue of 42 patients submitted to laparoscopy and hysteroscopy for the management of endometriosis. Sixteen of these patients were using oral contraceptives alone prior to hospital admission, while the remaining 26 were using them in combination with resveratrol. Inhibition of both aromatase and cyclo-oxygenase-2 expression was significantly greater in the eutopic endometrium of patients using combined drospirenone + resveratrol therapy compared with the endometrium of patients using oral contraceptives alone.

Is a Multispecies Probiotic Mixture Effective in Constipation During Pregnancy?

 

Twenty pregnant women with functional constipation received  a daily dose of Ecologic(R)Relief (Bifidobacterium bifidum W23, Bifidobacterium lactis W52, Bifidobacterium longum W108, Lactobacillus casei W79, Lactobacillus plantarum W62 and Lactobacillus rhamnosus W71 (total 4*109 CFU)) for 4 weeks in this pilot study.  At week 4 defecation frequency significantly increased from 3.1 at baseline to 6.7 in week four. Compared to baseline, there was a significant decrease in 1) sensation of anorectal obstruction from 90.0% to 45.0%  2) sensation of incomplete evacuation from 90.0% to 40.0%,  3) straining during defecation from 100% to 65%, 4) episodes of abdominal pain from 60% to 20%  and 5) the presence of reflux episodes from 60% to 20% in week four.

Is a multispecies probiotic mixture effective in constipation during pregnancy? 'A pilot study' 

 

Maternal Probiotic Supplementation During Pregnancy and Breast-feeding Reduces the Risk of Eczema in the Infant.

 

Two hundred and forty one mothers (with allergic disease and atopic sensitization)-infant pairs  were randomly assigned to receive  Lactobacillus rhamnosus LPR and Bifidobacterium longum BL999 (LPR+BL999), or L paracasei ST11 and B longum BL999 (ST11+BL999), or placebo, beginning 2 months before delivery and during the first 2 months of breast-feeding. The infants were followed until the age of 24 months. Skin prick tests were performed at the ages of 6, 12, and 24 months. The risk of developing eczema during the first 24 months of life was significantly reduced in infants of mothers receiving LPR+BL999 and ST11+BL999. The respective odd ratios for chronically persistent eczema were 0.30 and 0.17. Probiotics had no effect on the risk of atopic sensitization in the infants.

Docosahexaenoic Acid (DHA) for Reading, Cognition and Behavior in Children Aged 7-9 Years

 

Three hundred and sixy-two healthy children (7-9 years) who were under performing in reading were randomly assigned to 600 mg/day DHA (from algal oil) delivered in three 500 mg capsules per day, each providing 200 mg DHA or placebo  for 16 weeks. Significant effects were seen in the  subgroup of 224 children whose initial reading performance was ≤20(th) centime. Parent-rated behavior problems (ADHD-type symptoms) were significantly reduced by active treatment, but little or no effects were seen for either teacher-rated behaviour or working memory.

Docosahexaenoic Acid for Reading, Cognition and Behavior in Children Aged 7-9 Years: A Randomized, Controlled Trial (The DOLAB Study). 

 

Cholesterol-lowering Efficacy of Lactobacillus Plantarum in Hypercholesterolaemic Adults. 

Sixty hypercholesterolaemic volunteers (18-65 years) were randomly given one capsule daily containing 1·2 × 109 colony-forming units of Lactobacillus strains or placebo for 12 weeks.  A significant reduction of 13·6 % in plasma total cholesterol (TC) levels was observed after 12 weeks of consumption in the L. plantarum group when compared with the placebo group. Low initial values (LIV, 2000-2500 mg/l) v. high initial values (HIV, 2510-3000 mg/l) or total cholesterol were analysed. The present results showed that  L. plantarum  had a better effect in patients with higher levels of cholesterol.

The Effect of Black Currant Anthocyanins on Visual Field in Glaucoma.

Thirty-eight patients with open-angle glaucoma (OAG) were randomly assuaged to  black currant anthocyanins (BCACs) or placebo for 24 months. There was a statistically significant difference in the mean change from baseline in the  Humphrey visual field mean deviation (MD), the ocular blood flow increased in the BCAC group compared to the placebo group. There were no significant changes  observed in systemic and ocular conditions including  intraocular pressure (IOP). Oral administration of BCACs may be a safe and promising supplement for patients with OAG in addition to antiglaucoma medication.

Lactobacillus reuteri DSM 17938 for the Management of Infantile Colic in Breastfed Infants

 

Eighty infants aged < 5 months with infantile colic* who were exclusively or predominantly (>50%) breastfed were randomly assigned to receive  L reuteri DSM 17938 (10(8) colony-forming units 5 drops, 1 time daily or placebo for 21 days. The rate of responders was significantly higher in the probiotic group compared with the placebo group at day 7, day 14, day 21 and at day 28. Throughout the study period median crying time was significantly reduced in the probiotic group  compared to the control group.

Lactobacillus reuteri DSM 17938 for the Management of Infantile Colic in Breastfed Infants: A Randomized, Double-Blind, Placebo-Controlled  

 

Effect of Anti-inflammatory Supplementation with Whey Peptide and Exercise Therapy in Patients with COPD.

 

Thirty-six elderly stable COPD patients were randomly assigned to a whey peptide nutritional supplement or a placebo in addition to low-intensity exercise therapy.  The nutritional supplement was a whey peptide with anti-inflammatory and anti-oxidant effects, n-3 fatty acids, vitamin A, C and E enriched. The nutritional support group had a significant increase in  body weight, %IBW, FM, energy intake, %AC, Alb, PImax, PEmax, 6MWD, WBI, emotional function, and CRQ and the levels of hsCRP, IL-6, IL-8, and TNF-α were reduced significantly. There was no significant change noted noted in the control group.

Efficacy of Omega-3 fatty Acid Supplementation on Serum levels of Tumour Necrosis Factor-alpha, C-reactive Protein and Interleukin-2 in Type 2 Diabetes Mellitus Patients.

 

Eighty-four subjects aged 45-85 years with at least a two-year history of type 2 diabetes mellitus either received three omega-3 capsules per day (eicosapentaenoic acid 1,548 mg; docosahexaenoic acid 828 mg; other omega-3 fatty acids 338 mg) or placebo (sunflower oil 2,100 mg) for eight weeks. Serum IL-2 and TNF-α levels were significantly reduced in the treatment group compared to the controls however there was no significant change in serum CRP levels.

Antioxidant Supplement and Long-term Reduction of Recurrent Adenomas of the Large Bowel

 

Three-hundred and thirty patients who underwent polypectomy completed this study comparing antioxidant supplementation to placebo for 5 years and increased risk of adenoma recurrence. The antioxidant contained 200 μg selenium, 30 mg zinc, 2 mg vitamin A, 180 mg vitamin C, 30 mg vitamin E daily. At a median follow-up of 4 years 100 patients had recurrence: 38 in the intervention and 62 in the placebo arm. The 15-year cumulative incidence of recurrence was 48.3 % in the intervention and 64.5 % in the placebo arm. A 39 % reduction of the risk of recurrence was observed in the intervention compared to the placebo group the risk reduction was similar for small tubular and advanced adenomas.

Propionyl-L-carnitine Hydrochloride for Treatment of Mild to Moderate Colonic Inflammatory bowel Diseases.

 

Fourteen patents with mild to moderate ulcerative colitis (UC) or Crohn's disease (CD) colitis, with disease activity index (DAI) between 3 and 10 and under stable therapy with oral aminosalicylates, mercaptopurine or azathioprine, for at least 8 wk prior to baseline were given 2 g a day of Propionyl-L-carnitine. After treatment 71% of patients achieved response and 64% obtained remission. The reponse once was higher for CD (100%) with 75% achieving remission  then UC (60%) after therapy. The median DAI of 7  decreased to 2  after treatment. Only patients with UC showed a significant reduction of DAI, from a median 6.5 to 2  after treatment. In CD patients the differences in DAI reduction were not significant. Seventy-nine percent of patients showed improvement of HI of at least 1 point, while only one CD and two UC patients showed HI stability; none showed HI worsening.

Pycnogenol® improvements in asthma management.

 

Seventy-six patents with allergic (dust mite) asthma in controlled conditions were randomly assigned Pycnogenol® 50mg bid and  or inhaled corticosteroid or inha;ed corticosteroid alone for 6 months. An individual patient's asthma condition was graded in five steps based on the daily dosage of inhaled fluticasone propionate with step 1 indicating 0 µg and step 5 the maximum dose of 500 µg ICS twice daily. Fifty-five percent of the Pycnogenol group passed to a lower inhaled corticosteroid dose compared to only 6% in the inhaled corticosteroid alone group. In the Pycnogenol there was no passing to a higher inhaled corticosteroid dose, where as in the inhaled corticosteroid group 18.8%  required a higher dose. No serious adverse events were observed in both groups and tolerability of Pycnogenol® was very good.  In the Pycnogenol group, night-awakenings were less frequent, the number of days with PEF<80% were decreased, days with asthma score >1 were lower, requirement for salbutamol and additional asthma medication less frequent, and consultation of general practitioner and specialist required less commonly.

Pycnogenol® improvements in asthma management. 

 

Effect of Lactobacillus rhamnosus LGG® and Bifidobacterium animalis ssp. lactis BB-12® on Health-related Quality of Life in College Students Affected by Upper Respiratory Infections.

 

Two hundred and thirty one healthy college students living on campus were randomly assigned to receive placebo or probtiotic (daily dose of minimum 1 billion cfu of each Lactobacillus rhamnosus LGG® and Bifidobacterium animalis ssp. lactis BB-12®) for 12 weeks. The median duration of upper respiratory infection was shorter by 2 days and the severity score was significantly lower by 34% with probtiocs versus placebo. The number of missed days of work was similar in both groups but the number of missed school days was lower in the probiotic group.
