Recap Grant Year 3:
 
Over the past 3 years, in partnership with CDC's National DPP, AADE has contracted with 45 accredited Diabetes Self-Management Education (DSME) programs in 16 states to implement the National Diabetes Prevention Program (DPP). The more than 230 lifestyle coaches that were trained have reached over 2500 individuals identified with prediabetes and enrolled in the program. AADE DPP Sites have implemented over 230 DPP classes since 2013 and are seeing their DPP participants transform as they adopt new healthy lifestyle change behaviors.
 
All AADE DPP sites are pending DPRP recognition and a total of 10 sites have achieved full recognition so far. Sites have shown the capacity to deliver an effective lifestyle intervention that meets CDC's Diabetes Prevention Recognition Program (DPRP) Standards and Operating Procedures while facilitating a quality program.
 
AADE's focus in year 3 of the grant has been towards contributing to the overall sustainability of the National DPP by working with its network of AADE DPP sites. Due to AADE's increased work within the DSME landscape, sites have had the ability to leverage existing relationships with physicians and practice groups, increase outreach efforts to community-based organizations with an interest in delivering the DPP, focus on clinical-community linkages, and work to provide the National DPP to disparate populations.
 
AADE and DPP sites have begun to host meetings with employers and insurers across states to promote its business case for coverage of the National DPP. AADE DPP Regional Meetings have been conducted, bringing a variety of stakeholders together, ranging from government agencies, healthcare organizations, community groups, business coalition and AADE DPP sites, to discuss the National DPP and local efforts to scale and sustain the program.
 
In 2015, a panel group of diabetes prevention program champions in the field of DPP, presented at AADE15 Annual Meeting on the latest activities of the National Diabetes Prevention Program and what to consider in the current landscape in order to obtain DPP reimbursement. Other activities in regards to prevention at Annual Meeting included a Reimbursement and Sustainability training for all AADE DPP Site Program Coordinators and keynote speaker included Ann Albright, PhD, RD of CDC's Division of Diabetes Translation.
 
Efforts will continue to be made to scale and sustain the National DPP with our network of AADE DPP sites in Year 4 of the grant. AADE would like to contribute much of its success and activities to our 45 sites and congratulate them on a successful year!

Spotlight on AADE's DPP Team
Providence Hospital
Mobile, Alabama
 
The Diabetes Center at Providence Hospital in Mobile, AL is part of Ascension Health, which is the largest nonprofit health system in the U.S. with hospitals in 24 states, and the world's largest Catholic health system. Phyllus Justice, MPH, RD, CDE, manager of the Diabetes Center and Program Coordinator of DPP, has seen the prevalence of diabetes increase from 3.5% of the population in 1990 to its current rate of 13% in AL. Approximately 50% of the population now has diabetes or prediabetes, most undiagnosed. She was excited at the possibility of starting a Diabetes Prevention Program, in hopes of helping people achieve healthy lifestyles while avoiding or delaying the onset of diabetes.
 
Providence Hospital's first DPP cohort is currently in progress with 16 participants enrolled. Donna Woodmansee, RN, CDE, is the Lifestyle Coach and facilitator of the program. Overall, the DPP participants have collectively lost 72 pounds after only session 5 in the program!
 
To market the program, posters and brochures were made and distributed to physician's offices, local fitness centers and churches. Members of the Providence Hospital team met with area physicians to discuss the program and the potent to refer patients. 81% of the current DPP participants were from physician referrals, which included labs indicating FBG and A1c in the prediabetes range. Providence Hospital also spoke at the 2nd annual Diabesity Conference in Mobile, AL. Mary Honkanan, M.D., Internal Medicine Specialist at Providence, is one of the physicians who is actively referring patients to the program. She is a believer in aggressive treatment of prediabetes. She states that" treating prediabetes may be lifesaving and prevent heart attacks. Treating prediabetes will help prevent premature cardiovascular events, such as heart attack or stroke, in addition to preventing or delaying the onset of diabetes."
 
Different techniques and strategies have been utilized to keep participant engagement and retention in the program. Misfit Flash activity trackers and pedometers are given to each participant. Participant are encouraged to connect with each other and follow their progress on the app's social connection. Participants enjoy sharing each weeks experience with each other. They also enjoy getting information in "bite-size" amounts, which they say makes it easier to absorb and is more "doable."
 
One participant's states, "I have already spoken to co-workers about this program and one has started working with me on watching their fat intake. Someone told me just today that there was a lightness and happiness in my face. I'm smiling more!"
 
Lifestyle Coaches will continue to work to provide the support and resource needed to make life-long healthy behaviors. Participants will work towards achieving individual goals and monitoring their activity.
 
Ascension Health Smart Health insurance has recently approved the DPP as a Health Savings Account (HSA) benefit. According to the Internal Revenue Code 213, a qualified medical expense is defined as "medical care to include the diagnosis, cure, mitigation, treatment, or prevention of disease." To live within the ruling, a physician would have to diagnose the fact that the individual has prediabetes or is at risk of Type 2 diabetes. Other reimbursement strategies have included participants paying $200 for the full program or providing a quarterly payment option of $60.
 
Providence Hospital will continue to provide support to individuals in the community. Providing a high quality program implemented by trained Lifestyle Coaches and overseen by a diabetes educator.
  
  
  
American Pharmacist's Month
 
October is American Pharmacists' Month, which is sponsored by the American Pharmacists' Association (APhA) in recognition of "the critical role [that] pharmacists are playing to help improve medication adherence, increase immunization rates and to educate patients on the many changes that are taking place across the health care landscape." With the continually evolving landscape of health care and the health care team, pharmacists in every practice setting - community, hospital, industry and academia - are playing a more vital role in patient care and education. The American Association of Diabetes Educators would like to acknowledge and recognize this tremendous contribution, and highlight the significance of pharmacists and their role in diabetes education, management, prevention and support.
 
Pharmacists are in a unique position to impact the quality of life for many individuals living with diabetes or at-risk for developing type 2 diabetes. AADE's Diabetes Prevention Program is working with pharmacies in Iowa and Kentucky to implement the National Diabetes Prevention Program (DPP), a lifestyle change behavior designed to prevent or delay the onset of type 2 diabetes. With a focus on lifestyle modification, Hy-Vee (West Des Moines, IA) and Kroger Pharmacy (Lexington, KY) are playing vital roles as community pharmacies in delivering specific interventions designed to prevent the onset of diabetes, which in many cases, most people don't realize they are at-risk for.

National  Diabetes Month
  
Observed every November is National Diabetes Month, to draw attention to the millions of Americans who are affected by diabetes. The National Diabetes Education Program's (NDEP) 2015 theme is Diabetes Education and Support: Everyone Has a Role. What's Yours? The theme highlights the need for ongoing diabetes education and support among people with diabetes and those who care for them.
 
 
 
"DID YOU KNOW that Diabetes Prevention..."
National Diabetes Month 2015 is around the corner and AADE's Diabetes Prevention Program is gearing up to take part in the many planned activities at AADE to celebrate and recognize the fight towards treating and curing the disease. In particular, the Diabetes Prevention Program will be conducting a coordinated social media campaign. This campaign is designed to engage Members, DSME programs, as well as educate the public and potential stakeholders about the many misconceptions and unknown facts about diabetes prevention and the National Diabetes Prevention Program. We are calling this campaign "Did You Know." With an emphasis on highlighting the many key findings that punctuate the importance of the program, attention will also be focused on identifying the continued successes of the program at many of AADE's sites. The goal of this campaign is to generate attention and dialogue about the National Diabetes Prevention Program, AADE's diabetes prevention network and its continued goal towards improving reimbursement opportunities.

Sweet Home Chili on Spaghetti Squash
 
Adding a small amount of sugar to this dish softens the acidic taste of the tomatoes without too much sweetness. Spaghetti squash is jack-o-lantern.jpgalways fun to make and here it replaces pasta.
  
  
 
1 medium spaghetti squash (about 2 � lbs)
1 tablespoon canola oil
1 cup diced onions
1 cup diced green pepper
1 can (14. 5 ounces) stewed tomatoes
1 can (15.5 ounces) pinto beans, rinsed and drained
1 cup brewed coffee ( or 1 cup water = 1 teaspoon instant coffee granules)
2 teaspoons chili powder
1 teaspoon ground cumin, divided
1 � teaspoons sugar
� cup fat-free sour cream
� cup (2 ounces) shredded reduced-fat sharp cheddar cheese
2 -3 tablespoons chopped fresh cilantro leaves
 
 
  1. Poke several holes in the skin of the squash with a fork and microwave on high for 10 minutes. Squash should be tender when pierced with fork; if it isn't, microwave on high in 1-minute intervals until tender. Let sit to cool slightly. When squash is cool enough to handle, cut in half and discard seeds. Scrape inside with the fork to remove spaghetti-like strands. Transfer to a heat-proof serving dish and keep warm.
  2. Meanwhile, heat canola oil in a Dutch oven over medium-high heat. Add onions and pepper and cook for 5 minutes or until they begin to richly brown. Add tomatoes, beans, coffee, chili powder, and � teaspoon cumin. Bring to a boil over high heat. Reduce heat and simmer, covered, for 20 minutes or until tomatoes are tender and mixture has thickened. Break up any large pieces of tomato with the back of a spoon near the end of the cooking time. Remove from heat and add remaining � teaspoon cumin and sugar.
  3. To serve, place in bowls and top with chili, then sour cream, cheese, and cilantro.

Fresh Tip:  Roasted squash seeds are a healthy snack. To make them, wash the seeds and place them in an oven-proof dish sprayed with canola oil cooking spray. Season as you prefer, the roast for 7-10 minutes at 400 �F, cool, and serve.
