An Interview with Prof. Jukka Takala: Continuity is Key to Echocardiography in Hemodynamic Sepsis Management

"Now, this new technology offers the possibility of  getting the basic information with a quite minimal amount of training, actually. When we introduced the technology in our unit, most of our staff had no training in esophageal echocardiography. We found out that, in fact, just with short training of a few hours' duration, you could introduce the technology so that the investigations by the staff with these devices could be performed validly.
"We did a validation study such that we recorded the performance of the staff using a standard approach for inserting the probe and to get the information. This was then re-evaluated by a trained cardiologist, an echo specialist, and we found an excellent agreement between the information obtained by the cardiologist versus our staff.

The Use of hTEE™ to Guide Hemodynamic Management in a Patient Undergoing Scoliosis Surgery

A 67-year-old female patient was scheduled for posterior correction of degenerative scoliosis reaching from the third thoracic segment down to the iliac bone. Her medical history was devoid of any cardiac risk factors. However, her preoperative electrocardiogram showed signs of an older inferior myocardial infarction. The preoperative echocardiography was inconspicuous. Our case illustrates the use of hTEE™ to aid in diagnosing the correct cause of intraoperative hypotension. Due to its miniaturization, the hTEE™ could be safely used in prone position |
Moreover, newly trained users obtain consistent, actionable information - here is a graph presented at ESICM, Lisbon, 2012 (Merz TM, Cioccari L, Baur HR, Takala J. Management of Hemodynamically Compromised Patients using the ImaCor ClariTEE Probe - A feasibility study). Note the excellent agreement between newly trained users and experienced cardiologists in using hTEE to assess global left ventricular contractability using fractional area change.

Prof. Takala spoke solely on hTEE in the Gold Hall, where he presented in the session "Hemodynamic resuscitation in sepsis". At the pre-symposium, post-graduate hemodynamic monitoring course "Making the Right Measures", Michel Slama, MD, PhD from CHU Amiens Sud provided excellent insights on hTEE feasibility and the impact hTEE has on management from his multicenter study. I left the ISICEM energized and excited by the level of enthusiasm for our hTEE technology. I offer many thanks to this distinguished group of physicians who know first-hand the value of hTEE in the clinical setting. Our next hTEE symposium will be held at ISHLT (the International Society of Heart and Lung Transplantation) in Montreal Quebec on April 24th.
