Please note that after finalizing the ReproTech policy on specimens at risk for active disease transmission, additional revisions have been made to the Specimen Transfer Forms, Registration and Transfer From forms effective June 2, 2016.

Also ReproTech has revised the Protected Health Information Release Authorization forms. Users who download online RTL forms will now receive, for optional completion, a PHI Release Authorization to be completed if they wish disclosure of records by ReproTech to another party. Additionally, patients may be advised to download this Release form at www.rtlmedicalinforelease.com
