Transfer Authorization Form Changes

Effective June 3, 2013, ReproTech will be using UPS as our carrier for specimen shipments. We will no longer be using FedEx. We appreciate your cooperation in this transition in which we are confident will be seamless and beneficial for all.
Why we switched:
- Embryo shipping: UPS will ship embryos as an approved item
- Cost: ReproTech is able to continue to offer our current fees
- Ease of Returns: Same day pick up is available

IMA shipping insurance, starting at just $19.50 per shipment.
Changes to Transfer Authorization forms:
The Transfer TO ReproTech Authorization form: ONE form is now in use for ALL tissues, form AU ACQ 100.
The Transfer FROM ReproTech Authorization forms are now 3 pages and include:
1. an option to decline or choose shipping insurance
2. an option to choose number of shipping tanks
3. IMA Insurance flyer
Forms BJ DIS 100 and BK DIS 100 replace all previous Transfer FROM forms.
Please contact your ReproTech office to obtain copies of these forms.

FDA Warning letter posted

Do you properly label your SIP specimens? The FDA issued a Warning letter for lack of Labeling of SIP specimens and Donor Eligibility violations to a Washington fertility center on April 24, 2013.

RTL Staff Changes

As you know, our President, Russ, retired on May 31. RTL is excited about the new changes within our company.
We are pleased to announce that Brent Hazelrigg will be assuming the role of President, and continue as CEO.
Our FPN Manager, Joy Bader, will be taking on the role of Vice President of Operations.
Congratulations to Courtney Dennewitz, of RTL-NV, who will be taking over the FPN Manager position. Courtney will assist RTL-NV with its transition and training of its new Customer Services Rep, and she will begin the FPN Manager job once training is complete.
Welcome to James, our newest RTL team member. James is our RTL-TX Client Services Representative.
