Dear Parents,

We hope your summer has been filled with making memories with your children. Some of you are anxious for school to start so you can begin a routine once again. Some of you are dreading the day when the children go back to school because you have enjoyed them so much.  And probably most of you have a combination of feelings.  

Here at Notre Dame we are excited for the children to come back and fill our halls with energy.  We miss them!!! Your loss is our gain.  We have been feverishly getting ready for the children and the beginning of school. One of the items that need to get done are the dreaded forms that need to be filled out.

Below please find a list of forms that need to be filled out for the beginning of the school year.  We are asking that you complete all the applicable forms and return them to us by no later than Friday, August 29th.

If you have problems printing these forms they will also be available in each of the school offices.

We will be having 2 days specifically for you to turn in your forms:

We chose these dates because of the special events taking place on those evenings. You are also welcome to drop them off any time before the above dates at one of the school offices before August 29th.

There is one more form that we could not send via email and that is the pink student emergency form.  Those will be available on the dates above and at the each school office or if you stop in ahead of time.

• Home and School Family Dues 2014-2015(One per family, required)
• Immunization Form(online) or Immunization Form(printable) (One per child, required)
• Parent-Guardian Communication 2014-2015 (One per family, required)
• Parent Handbook Verification form 2014-2015(One per family, required)Please read the Parent Handbook below before signing.
• Permission to Photograph Form 2014-2015(One per child, required)
• SCRIP Release Form 2014-2015 (One per family, required)
• Student Emergency Form 2014-2015 (One per family, required)
• Technology Acceptable Use Policy 2014-2015 (One per family, required)

• Free & Reduced Lunch Letter and Application 2014-2015 (One per family, If needed)
• Medication Consent Form 2014-2015 (One per child, if needed)
• Medical Evaluation Form 2014-2015 (Preschool families only)
• Physician's Authorization of Medication Form 2014-2015 (One per child, if needed)
