According to SHRM, national HR organization that focuses on key HR issues, they stress what I always do, even if you are small four employees and under, you are mistaken if you think an employee handbook isn't necessary.  
Here are a just a few areas that require your immediate attention for changes in your handbooks:

NLRB, free speech rights
.  These rights extend to social media sights, as it relates to pay, working conditions and unpopular bosses.

Social Media and Data Privacy..your handbook must be clear on the fact that employees have no right of privacy while accessing social media  at work or on company-owned equipment.  Handbooks need to be clear on prohibiting employees from discussing proprietary information as well. 

Reasonable accommodations employees are able to request need to be clear in such handbooks as well including the process on how to request one and the criteria that will be considered.

Retaliation is not to be tolerated in the workplace.  Be sure this is clear in the handbook that references various policies it applies to such as those where employees can lodge complaints, sexual harassment, workplace bullying, whistleblowing as a few examples.

Wages and Payroll, all policies related need to be clearly spelled out in handbooks.
