Value employee input. It is the "front line" that knows the "real" dynamics of day to day functioning. Why would you want those that know best to offer solutions, suggestions and most importantly, identifying challenges they face in order to make your business/organization look good?

Brainstorming as to challenges within homogeneous groups allows all to contribute from their perspective, as to what is holding them back. Together, we identify possible solutions, with goals attached in order to have measurable items and keep all focused. Such brainstorming includes discussion on Leadership needs, resources, work processes and developmental needs to name some key items.
From the planning groups, tangible, formal, written plans are put in place ensuring the vision and mission..keeping all focused appropriately.
The process of change is difficult before it gets better. There is heightened sensitivity as it requires individuals to make changes. Change is hard but well worth it!
Results for my clients have been culture changing and sustainable but that only comes with complete commitment by all members of management. Supporting change is a process..processes are continuous. Implementing new strategies, measuring performance regularly and objectively. Ensuring all policies are up to date and clearly written and having functional job descriptions all contribute to moving toward effective organization change.
At the very least, I would recommend you "scan" your work environment to determine employee morale, conflicts that may exist and meetings that are ineffective. Those are the first signs that larger changes needs to be a focus.
Your organization will not "magically" change...it takes commitment, changes in individuals and group dynamics, leadership development, follow through and support at all levels.
