A Little Help Over Here? Technical Assistance Linked to Successful Primary Care Transformation

The green line indicates significant primary care quality improvement gains following eight technical assistance visits. 

Higher levels of technical assistance are associated with improved quality of primary care, according to a study in the January 2013 issue of Health Affairs.  

 

Data from the NYC Primary Care Information Project (PCIP) showed that physicians who had eight technical assistance training (TA) visits and nine to 24 months of exposure to PCIP assistance made significant quality improvement gains in EHR-sensitive quality of care measures (e.g. breast cancer, diabetes, chlamydia and colorectal cancer screenings).

An annual primary care celebration, PCDC's Spring Gala hosts more than 600 leaders from healthcare, banking, philanthropy, government and other sectors to raise critical funds to support PCDC's programs that expand and strengthen access to primary care in underserved communities.  PCDC's Spring Gala will be held at Pier Sixty at Chelsea Piers on Manhattan's West Side.

Patient-Centered Medical Home Conference Unites Stakeholders, Looks to the Future
Where does the medical home movement stand in New York State? Where are we strong, what are the challenges, and how will we move ahead? Joining with the United Hospital Fund and the New York State Chapter of the American College of Physicians, PCDC recently hosted Moving New York's Medical Home Forward: Spread and Sustainability, the second annual conference on the status of patient-centered medical homes (PCMHs) in New York State. 

Following a 42% increase in the number of NCQA-recognized medical homes in New York State between 2011-2012, conference participants assessed the successes and challenges facing PCMH, and worked to develop an agenda for advancing PCMH and other primary care innovations.     

Featured speakers included NYS Health Commissioner Nirav Shah, MD, Acting NYS Mental Health Commissioner Kristin Woodlock, RN, Jill Hummel, WellPoint VP of Payment Innovation, Dennis Weaver, MD, CMO of Southwind, Hal Teitelbaum, MD, CEO of Crystal Run Healthcare, and Foster Gesten, MD, Medical Director of the NYS DOH Office of Quality and Patient Safety.

The conference will address a wide range of topics for organizations exploring innovative evidence-based facility design solutions, planning and financing opportunities, and community engagement and collaboration strategies.

Launched by Morgan Stanley, The Kresge Foundation and the Local Initiatives Support Corporation (LISC), the Healthy Futures Fund was established partially as a response to the Affordable Care Act and the 20 million new health care consumers that the legislation is likely to create.

Crescent City Beacon Health Center Trained by PCDC Named "Exemplar Practice"

Daughters of Charity Services, an innovative New Orleans health center network and PCDC partner, was recently identified as an "exemplar practice" by the Robert Wood Johnson Foundation.
 
The Primary Care Team: Learning from Effective Ambulatory Practices (LEAP) identifies and shares staffing arrangements that make primary care more accessible and effective for patients. Daughters of Charity is one of up to 30 practices being selected for this distinction out of hundreds that were nominated.

Through a partnership with the Crescent City Beacon Community, PCDC has worked with Daughters of Charity and 18 other practices to help strengthen this care team model.
