When Jesus came to his family, they had rejected him.  Saying you must stay home, you are crazy for going around healing people.  Jesus was then driven out to the water by the townsfolk claiming the same thing as his own blood.  Thinking of his followers he stated here are my mother, my father, and my brothers.  Those who follow the will of God are truly my family.  He was trying to say that your family might not act like your family as your closer friends might.
