iPAD INFORMATION
As most of you know, St. Francis DeSales High School is headed into the second year of our iPad one to one program in the 2014-2015 school year.  The deployment for students new to SFDHS will begin during the month of June.  This will include incoming Freshmen and Transfer Students. Below is information related to the iPad roll-out.  If new information needs to be disseminated, we will send updates. For now it is important for you to read the documents included here for several reasons:
  • So new students & their parents will be aware of the date and times that they are scheduled to come to St. Francis DeSales High School to get a brief orientation and pick up the iPad.
  • So new students and parents can learn and understand the Acceptable Use Policy for technology in general and for the iPads specifically.
  • So new students & parents can get the iPad Responsibility Form that MUST BE BROUGHT WITH YOU FILLED OUT AND SIGNED BY BOTH THE STUDENT AND A PARENT/GUARDIAN WHEN YOU COME TO THE IPAD ORIENTATION DURING THE SUMMER.  IF YOU DO NOT HAVE THE FORM WE CAN GIVE YOU ONE BUT YOU CANNOT GET YOUR IPAD WITHOUT FIRST SIGNING AND TURNING IN THIS FORM. The form is available below.
Please see the list of documents and their descriptions below.
PRE-PARTICIPATION PHYSICALS FOR ALL STUDENT-ATHLETES & INCOMING FRESHMEN

New this year: Student-athletes must submit medical history online; a printed copy of this material is taken to the doctor to complete the physical. Directions on how to create the medical history page is located below; it on the school's athletic web page under Forms as well.

All freshmen must have a completed physical form turned in by the first day of school. You will use the same online medical history/physical form that our student-athletes complete. Step-by-step directions on setting up a medical history page (which you will take to your doctor to complete the physical) is located below and on the school's athletic web page under Forms.

PrivIT e-PPE Instructions (this may take 15 minutes):

  • 1. Go to http://www.e-ppe.com and type in your child's high school (St. Francis DeSales High School) in the search box.
  • 2. From the results page, click on the name of your child's high school.
  • 3. From the landing page, click Register.
  • 4. Fill out the registration information. This will be the email address and password you will use to access your child's PrivIT e-PPE in the future. Click Register.
  • 5. Complete your Personal Details/Demographics section. There are six components to this area (Personal Information, Primary Insurance, Next of Kin, etc). IMPORTANT: By completing this section your demographic information can be copied to pre-populate your child's PrivIT e-PPE demographic information from the Manage Family Members page.
  • 6. On the Home page under Manage Family click the Manage Family Members button.
    • Click Add New Member.
    • Enter your child's first name (and last name if different).
    • Click Add
    • On the next screen you are able to copy PrivIT e-PPE Family History data and demographic data from other family members.
    • Confirm the information you want copied and click Copy.
You will now see your child listed under Family Members and you can begin completing your child's PrivIT e-PPE.
To begin completing your child's PrivIT e-PPE click on the pencil in the PrivIT e-PPE Questionnaire column next to their name. A message will appear notifying you that you will be editing information that will be associated with a particular high school or organization. Click Ok.
Complete the demographic information section and click Save and Exit.
You will be taken to the introduction page for the PrivIT e-PPE Questionnaire. Click Start.

Complete the 17 components of the e-PPE questionnaire. Although we recommend completing the questionnaire all together at once, you are able to "Save and Exit" to allow you to finish at a later date if needed. IMPORTANT: In order for a component to be 100% complete you will need to answer all mandatory questions, marked with a red asterisk, and check the box at the bottom that states "I have thoroughly read all questions on this page and answered them to the best of my knowledge." You will not be able to print your forms until this step is complete.

  • 7. Click Finished.

After completing your child's PrivIT e-PPE, you MUST add your child to the teams he/she will be participating in or trying out for this academic year. To do this, click Manage Your Team Memberships from the home screen and check the box next to the sports your child will be playing.

IMPORTANT: You cannot skip this step as it is critical for you to be able to print your child's reports to take to the physical exam. In addition, your child's coaches and athletic trainers will be able to review eligibility status, emergency contact information, and see your child on the team roster. INCOMING FRESHMEN: There is a box that you check for being a freshman.

  • 8. After you have completed your child's PrivIT e-PPE and added them to a team you will need to print out the appropriate reports/forms your child will need for his/her physical exam.
  • 9. Once the physical is completed, you can turn it in to your child's coach (FRESHMEN: Please give it to your homeroom teacher) or scan and upload it to your child's profile.

*STUDENT-ATHLETES ONLY: All student-athletes who complete their physicals provided by Ortho-Neuro on July 16 will have their physical scanned and uploaded by the school's Athletic Trainer. Click here for flier on the Ortho-Neuro Physical day

SUMMER READING
Students are required to read this book by the first day of school.
I Am Malala: The Girl Who Stood Up for Education and Was Shot by the Taliban by Malala Yousafzai with Christina Lamb

ABOUT THE BOOK: When the Taliban took control of the Swat Valley in Pakistan, one girl spoke out. Malala Yousafzai refused to be silenced and fought for her right to an education. When she was fifteen, she was shot in the head at point-blank range while riding the bus home from school, and few expected her to survive. Instead, Malala's miraculous recovery has taken her on an extraordinary journey from a remote valley in northern Pakistan to the halls of the United Nations in New York. At sixteen, she has become a global symbol of peaceful protest and the youngest nominee ever for the Nobel Peace Prize.

FYI: The iTunes store has it for $6.99 for download.

DAILY REFLECTION
Very often we do not abandon ourselves entirely into the hands of God because it seems to us that God will not take very much notice of us, since we consider ourselves good for nothing. The world like-wise has little use for us. On the contrary, a courageous spirit relies on faith, grows in courage amid difficulties, and knows quite well that God loves, supports and helps those in need when they confide and hope in Him. (Letters 1197; O. XVII, pp. 205-206)
