If you don't have the member's identification number and cannot access our secure site, speak with a representative at 1.888.324.2064 from 7 a.m. to 6 p.m., Monday-Friday.

 

You participate with CoOportunity Health through Midlands Choice.

 

CoOportunity Health contracts with Midlands Choice for provider network and credentialing services.

 

As a Midlands Choice contracting provider, you participate in all or most of CoOportunity Health's networks.

CoOportunity Health contracts with HealthPartners Administrators, Inc. to provide claims processing, medical management and certain other administrative services on our behalf.

 

Because you will not file our claims with Midlands Choice, you should enroll with HealthPartners for your Electronic Data Interchange [EDI] needs. This enrollment is for electronic transactions only.

 

You'll find clearinghouse information and Payer IDs on our Claims Submission page at coOportunityhealth.com. Please confirm this information with your vendor or clearinghouse prior to submission. Even if your clearinghouse is not listed, your vendor can redirect claims through a HealthPartners approved clearinghouse.

 

Iowa Marketplace Choice Plan [IMCP] members are not assigned to a specific provider.

 

Our Iowa Marketplace Choice Plan is an open-access plan. Covered members can access any provider listed under our Iowa Marketplace Choice Plan directory and are not assigned to a specific provider. For more information, visit our Plan Resources page and select the  
IMCP tab.

  3. Register for access to our secure Provider website to review member benefits and eligibility, claims status, and remittances.
