1. Proof of Citizenship (Passport or Social Security Card)

2. Proof of Residency (Driver's License, Residential Lease or Utility Bill)

3. Proof of Household Income (W2 Forms, Pay Stubs, Unemployment/Disability, Social Security/Pension Retirement Income, or 2012 Taxes)

4. Health Insurance (if you are currently covered)

5. Immigration Status/Legal Resident (Document Number of Legal Status)

6. Form of Payment (Personal Check, Money Order, Cashier's Check or Credit Card)
