Funding Opportunity to Advance Your Initiative

Experiencing a hurdle or challenge in your community health improvement initiative? Need to engage new partners and expand your efforts to increase your success and improve health for more people in your community? Looking to raise awareness and support for your community health improvement initiative?

Change Incubator Funding aims to invest in community-based partnerships to overcome challenges and increase their readiness for and effectiveness in carrying out larger community health improvement initiatives. Funded partnerships can:
  • Obtain dedicated funds for strategic planning processes to overcome barriers and develop a product to immediately turn into action within a larger community health improvement initiative
  • Gain dedicated time for partnership development and collaborative work to advance a community health improvement initiative
  • Connect with HWPP staff and external consultants to build capacity and identify future funding opportunities

Seeking Community Mentors and Mentees for New Funding Opportunity

HWPP is seeking community-based partners working to improve the health of their communities to connect with other communities across Wisconsin to share about work, learn together, build relationships, and expand and enhance both communities' efforts to improve health. Partners are sought to serve as mentors and mentees:
  • Mentee partnerships - for those who are forming a new multi-sector partnership or launching a new evidence-based community health improvement initiative and seek to strengthen their partnership and organizational structure and increase local community leadership and readiness to implement, evaluate and sustain a community health improvement initiative
  • Mentor partnerships - for those who have a strong existing partnership and have successfully implemented an evidence-based community health improvement initiative and are looking to disseminate their work and explore opportunities to expand it to other Wisconsin communities

Reminder: Policy, Systems and Environmental Change Applications due January 8, 2016

Applications for Responsive Component - grassroots policy, systems and environmental funding  - are due January 8th.

Reminder: Behavioral Health Applications Due February 1, 2016 

The application deadline for the HWPP Strategic component with a focus on community-based behavioral health is February 1, 2016.
 
The initiative is calling for Community Coalitions to address and improve the behavioral health of Wisconsin communities. The initiative will provide up to $1.5M per community over the 8-year funding period. Click here to view the Request for Proposals (RFP).

Violence Prevention Initiative

The Violence Prevention Initiative (VPI) was a special funding initiative of the MCW Consortium on Public and Community Health through the Advancing a Healthier Wisconsin Endowment's Healthier Wisconsin Partnership Program. When the VPI funding period closed at its natural end date, the Consortium enlisted an external evaluator to assess the activities that had occurred during the funding period and provide structure for moving forward.
