Award Announcements
The Healthier Wisconsin Partnership Program, along with the MCW Consortium on Public and Community Health are pleased to announce that as of Friday, October 26, 2012, funding in the amount of $4,536,945 is being awarded to twelve community-academic partnership projects including:
- Four (4), 5-year Impact Awards providing funding for up to $750,00 each, and
- Eight (8), 2-year Development Awards providing funding for up to $200,000 each.
These partnerships have committed to advance the vision of HWPP to improve the health of Wisconsin residents. A total of 60 community organizations and 26 MCW faculty and staff members are partners on these projects. Seven of the projects are focused in urban areas (Dane, Milwaukee and Waukeshacounties), three in rural Wisconsin (Calumet, La Crosse, Outagamie, Rusk, and Winnebago counties) and two projects are statewide initiatives.
Community and MCW partners submitted 55 letters of intent. Of these, 27 were invited to submit full proposals and underwent competitive review by a National Merit Review Panel, consisting of both community and academic perspectives, as well as by the MCW Consortium on Public and Community Health.

| Clinic Model Recognized for Managing HTN | Published in Journal of Primary Care & Community Health
Community-based Chronic Disease Management (CCDM), a Impact Award from the 6th funding cycle of HWPP, uses alternative models of treatment and management of chronic diseases for uninsured patients in Milwaukee. A pilot study of different ways to address hypertension in this population was recently published in the Journal of Primary Care & Community Health, where 45% of the participants were able to reach national benchmark goals for blood pressure.
Journal authors acknowledge that further study is needed to evaluate this approach to managing hypertension, but are optimistic that this model may be translated to additional populations. Partners include Bill Solberg, MSW, LCSW of Columbia-St. Mary's and James Sanders, MD, MPH, Department of Family and Community Medicine at MCW.

Violence Prevention Initiative (VPI)

Project Updates
VPI is a community-based initiative that aims to: - decrease rates of violence in identified areas of Milwaukee and, possibly other areas of Wisconsin, and
- strengthen community capacity to prevent future violence.

Community Partner Teams
Two Community Partner Teams began programming in November 2010: Holton Youth and Family Center Collaborative (HYFCC)Two years into their effort, the collaborative continues to see substantive, steady growth in numbers served by the Holton Youth and Family Center. Since reopening in Fall 2011, the center has served 545 youth coming from 58 area schools through tutoring, literacy, health and nutrition, gender-specific and nurturing parent programming as well as youth leadership training. United Neighborhood Centers of Milwaukee (UNCOM) Violence Prevention Initiative UNCOM's outreach and partnering efforts have increased its reach in the community and success toward accomplishing its goals. UNCOM collaborated with MPS middle and high schools to provide programming to aid students in developing resiliency skills through a train-the-trainer model. In addition, project staff have been trained in two additional programs that will aid in enhancing youth communication skills, reducing stress, anger, anxiety and depression and improving learning ability. Two additional Community Partner Teams were selected through an additional Call for Partnerships, and began programming in July 2012:- Ripple Effect Milwaukee (REM): Spreading Peace and Building Communities
- Safe Schools Healthy Students Lindsay Heights Initiative

Injury Research Center at MCW

10 Year Anniversary Report
The Injury Research Center (IRC) at the Medical College of Wisconsin recently published a "10 Year Anniversary Report" highlighting the achievements and contributions made by its affiliated faculty, staff and partners to the field of injury prevention and control.
Injuries and violence are the leading cause of death in Wisconsin and the United States for people, ages 1-44 years. The report spotlights the IRC's efforts to create safer environments and products, support legislation to protect children and families, provide information to the scientific community about ways to reduce the risks of injury and violence, and optimize treatment and care for injured patients.
MCW's leadership, along with community and academic partners have played an instrumental role in helping to support the mission of the IRC. While significant contributions have been made over the past 10 years, much work remains to make the treatment and prevention of injury and violence a premier public health achievement.

RFP Release
The MCW Consortium on Public and Community Health anticipates releasing a Request for Proposals of the 9th Funding Cycle of HWPP in the first quarter of 2013. Additional information will be found on the HWPP website as it becomes available. Please contact HWPP with any questions.
We strongly encourage you to begin forming your partnership well in advance of submitting a request for funding. Community partners seeking MCW academic partners may find MCW's Faculty Collaboration Database (FCD) helpful. The search feature of the system is simple to use, yet extensive in the presentation of the results. To access this online database, visit http://fcd.mcw.edu, enter a key term into the Search function. Basic demographic information about MCW faculty as well as grants MCW faculty are involved with will be presented as well as other information such as published articles, leadership positions, educational and clinical expertise, community partnerships and more.
