Required School Forms

If you haven't already returned required school forms, please do so now. Please follow the instructions on each form and return them with your child to the school office.
 
 
Our School Forms Packet includes the following:
  • School calendar
  • Permission release forms
  • Health and dental forms*
  • REAP, after-school program forms
  • Personalized contact information form
  • Parents' Association membership and volunteer forms

The following school supplies will be provided by Summit and distributed to each student on the first day of school:

     2) Loose-leaf paper                          6) Homework folder
     3) Dividers                                      7) Schedule folder

Parent Contact & Emergency Information

 






Please remember to inform the school of any changes to your contact information, including your address, telephone numbers, and email addresses. Call the school office directly to make these changes.
 
In the event of inclement weather or an emergency, we will use our K12 Alerts service to communicate with you via voice message, text message, and email. Parents are responsible for the accuracy of the information in this system.
  
Returning families should log in to their K12 Alerts account and verify the accuracy of their contact data.
 
New Parents: You will receive your K12 Alerts account information via USPS next week. Please follow the instructions to log in and update your account. Parents are responsible for the accuracy of the information in this account.

REAP: Recreational Enrichment After-School Program  

Providing the opportunity to have fun, while learning through new activities and socializing with your peers, is the main goal of Summit's after-school program.
