TOGETHER
SHAPING THE FUTURE

Welcome to our August Newsletter!

This month we feature an interview with a professional fiduciary here in San Diego, Susan Fox. We've enjoyed working with Susan, admire her work and wanted to introduce her to you as well.  Susan shares very important information with our readers this month as her interview focuses on clearly defining what an Advance Health Care Directive is and why it is critical to have in place for you and your loved ones.  A fiduciary's work is significant since it encompasses both financial, business and personal aspects of our client's lives.

In this month's edition I also get to extol the virtues of working with an assistant care manager. This is a unique role I developed over ten years ago. Our longtime assistant care manager Mindy Waldhauser, MA, has worked with Grace Care Management faithfully over this past decade and she is very special to our team and clients. We could not do what we do without her!

It is my strong sense that the assistant care management model will begin to take hold and spread rapidly as a creative and common sense response to the overwhelming need for care management in our country.

Thank you again for giving us time to share our thoughts with you and we hope you leave feeling inspired until next time.

Blessings,

ASSISTANT CARE MANAGEMENT: INNOVATIONS IN HOME CARE
By: Cindy Hasz LVN, CMC
I believe geriatric nurse care management is changing and adapting to better suit the needs of elders and their families. In an ever increasing long term care market, the models of care delivery to the elderly and disabled have to keep pace with the increasing demands and higher expectations of a more educated health care consumer.

Elder care consumers have become quite savvy. The vast majority of them are baby boomers and adult children who are learning, many times through painful personal experience with family and friends, that our current "home care" models don't fit the needs of their loved ones.

They are learning that many times one must hire their own team of experts and pay privately if they wish to obtain any quality and retain any control over their families care in a broken system. The rapid growth in the concierge model of primary care reflects this shift.

INTERVIEW WITH SUSAN K. FOX
Licensed Fiduciary, Liberty Fiduciary Services Inc.

Susan K. Fox, the Principal and Founder of Liberty Fiduciary Services, Inc. has more than fifteen years of experience in providing direct services to vulnerable individuals, including those with Parkinson's Disease. Susan is a University of Michigan graduate and holds certificates from Cal State Fullerton in Professional Fiduciary Management, Trusts, and Conservatorships. She also earned certificates from UCSD Extension in Gerontological Counseling and Program Management, Counseling and Interpersonal Skills, and Geriatric Activities. She studied Reality Therapy and attained the Advanced Certificate from Dr. William Glasser of the William Glasser Institute.

What influenced you most to become a fiduciary and serve vulnerable individuals?

The combination of seeing a video of Mother Teresa's work with the homeless and forgotten along with the knowledge that approximately one in four people over 65 years old in San Diego County have no one that they can trust to speak for them or manage their affairs when they cannot do so. This statistic includes people who have millions, of dollars to the people who are on welfare and might receive as little as $600 per month to live on. There is a humanitarian need that can be filled by licensed, trustworthy and responsible professionals.

Where are you from and/or what is your background?

I was born and raised in San Diego and come from a good family who I love dearly. My pride and joy is my daughter, Eden, who is now 18. I spent nearly 20 years in banking, specializing in loan lending and auditing. I enjoy spending time with family and friends and my church family as well. Currently, I am focused on my health and fitness and have goals I want to reach within the year. I have often been told I have the gift of compassion and the ability to make a difference in and for all situations.
