Important update for the CANS assessment
Please read below for important updates from Smoky's Care Management Team.
Child and Adolescent Needs Survey (CANS)
Full implementation of the use of the CANS early childhood assessment tool has been postponed until March 1. Smoky has received information from the CANS developer that will help the network implement more efficient use of this instrument.
Please note that the comprehensive version of the CANS is broader in scope than needed for children ages 0 to 6. Providers should train on the currently posted 0-4 tool, which the developer reports will be updated on the website to indicate an age range through 6 years. The staff person who administers the CANS is not required to be a licensed clinician, but he or she must have completed the online training and passed the online training test. Additional details and instructions will be provided in upcoming bulletins.
I/DD Service Plan submission requirements
Note that (b)(3) and state-funded services require submission of the N.C. Department of Health and Human Services (DHHS) person-centered plan (PCP) template (see below). Do not use the Individual Support Plan format, which is limited to Innovations participants. The documents below were updated February 3, 2010.
There should be only one date on the plan coinciding with the date the qualified professional signed the plan. See page 22 of the Person-Centered Planning Instruction Manual for more information. State-funded service plans should be submitted with the addition of a signature page.
Attention: IPS-supported employment providers
Effective February 15, 2016, providers who provide Fidelity IPS-Supported Employment should begin submitting a copy of the DHHS Transition to Community Living Initiative (TCLI) Fidelity IPS-Supported Employment (IPS-SE) Participant ACH In/At Risk Checklist with Initial Service Authorization Requests (SARs) for mental health/substance use (b)(3) supported employment. This will help Smoky gather information more efficiently. These checklists do not need to be submitted for concurrent requests.
Co-occurring diagnosis and treatment
Remember that the focus of treatment should relate to the primary diagnosis and not be based on the provider specialty. Therefore, when an I/DD provider completes an assessment for an individual with a co-occurring disability, and the primary treatment need is found to be depression, the primary diagnosis would be the mental health diagnosis, not I/DD. Requirements for outpatient opioid treatment requests
Effective February 15, 2016, all requests for outpatient opioid treatment should include a copy of the most recent person-centered plan and comprehensive crisis plan update. For initial services requests, include a copy of the service order.
