Challenges in provision of integrated and collaborative care are many, but the rewards for true collaboration between behavioral healthcare and primary care have been demonstrated in both national and regional pilots.
Objectives:- Identify existing conceptual models of integrated care
- Review local adaptation of models of integrated care
- Identify strategies to implement a model of integrated care in a practice
