 Attention providers of Innovations Home and Community Based Services (HCBS)
The statewide HCBS self-assessment ends today, September 15, 2015, at 8:00 p.m.
Providers who are experiencing technical issues with the assessment website should email Smoky (HCBS@smokymountaincenter.com) before tonight's deadline.
Providers who are not experiencing technical difficulties and choose not to complete the assessment must submit a transition plan to Smoky.
Please note: If you are a provider of (b)(3) services, the Department of Health and Human Services (DHHS) has extended the deadline for self-assessment submissions through October 15, 2015. The HCBS site will become inactive after 8:00 p.m. today. It will be reactivated on or before 12:00 p.m. on September 16, 2015.
