Electronic Self-Assessment Launched for NC Home and Community Based Services Providers
The NC Department of Health and Human Services (DHHS) has launched the electronic self-assessment for Home and Community Based Services (HCBS) providers. The e-assessment must be completed by agencies that provide the following NC Innovations services:
- Day Supports
- Residential Supports
- Supported Employment
The e-assessment is also required for providers of Adult Day Health services offered through other Community Alternative Programs.
The e-assessment is mandatory and must be completed by September 15, 2015. Detailed instructions and the e-assessment can be found on the DHHS website. If you have questions about the e-assessment, please email hcbs@smokymountaincenter.com. |
