Reminder: 2015 Provider Needs Assessment Survey

Please take a few minutes to complete Smoky Mountain LME/MCO's Provider Needs Assessment Survey.
Smoky Mountain LME/MCO (Smoky) is conducting an assessment throughout its 23-county service area to identify the mental health, substance use and intellectual/developmental disability service needs of people who are uninsured, underinsured or covered by Medicaid insurance.
* Please use the "Provider Needs Assessment Survey" link on our Service Provider page to complete the survey.
Help Smoky plan for the future. Your input will help Smoky develop a plan to ensure an adequate provider network is in place to offer services in areas accessible to the residents the system is designed to serve.
