On May 8, 2015, members of the Home and Community-Based Services (HCBS) Transition Plan and strategic workgroup trained providers participating in the HCBS self-assessment pilot. Participating providers received the assessment in an electronic format. The e-assessment went live Monday, May 11. The pilot dates are from Monday, May 11, through Sunday, May 24. The pilot will inform statewide implementation of the e-assessment.

 

Smoky has created a dedicated email for questions about this pilot and the HCBS Transition Plan. All questions and comments may be sent to hcbs@smokymountaincenter.com.
