Question: What is the biggest mistake you see people making on the 5 Day Pouch Test?

Kaye's Answer: Not learning and following the plan or changing the plan drastically.

Learn the plan. Read the plan in full and be sure you understand it. Read the plan completely in order to understand the progression of your diet from Day 1 to Day 5. Pay close attention to understanding the liquid restrictions and slider foods: these are the most common problem areas that lead to weight regain after weight loss surgery.  As you become familiar with the 5 Day Pouch Test compare it to the early dietary stages following your weight loss surgery. Think back to what worked for you then and imagine the same will work for you again. Remember, you already know how to lose weight using your surgical tool. The effort you put into the 5DPT will return you to that place of healthy and reasonable weight management using your tool.

Try not to make too many changes to the plan. Keep it at 5 Days. Don't restrict foods and go hungry. Really pay attention to what changes you are making and constantly think about the Four Rules can be your way of life, not just a temporary means of taking off a few pounds only to put it back later. This is a way of life we agreed too when undergoing surgery and it is for our lasting health. The 5DPT can help you get back to doing what works with WLS to support your lasting health and weight goals.

Strive to Learn, Avoid the "Perfection Trap"    
If we complete the 5 Day Pouch Test having learned a few things about our self, our weight loss surgery and our capability in managing life and weight loss surgery in relationship to one another we have succeeded: we can deem the 5 Day Pouch Test a success. Here are some things to consider while treating the 5DPT as a learning experience:
 
-- What can I eat that gives my pouch a feeling of fullness? What do I eat that fails to give my pouch a feeling of fullness?

-- Have the liquid restrictions become automatic to me? Do I have heightened awareness of how I consume liquids with my meals and snacks?

-- Am I eating protein in a ratio of 2 bites protein to 1 bite complex carbohydrate? (2B/1B Rhythm)

If you follow the 5DPT you know I always say don't do the plan for quick weight loss. Do the plan to get back on track with our WLS basics and refresh your memory about the Four Rules, slider foods, and liquid restrictions. Any weight loss during the plan is a bonus. I stand by this.

In this special edition of our monthly 5 Day Pouch Test Bulletin we share the top most frequently asked questions we have received in the first half of 2013. While there are visible immediate rewards to doing the 5DPT such as increased energy and notable weight loss, the thing that matters after Day 5 is what we learned along the way.

The 5DPT as it is written and presented online and in print is an organized approach to getting back to the WLS basics both mentally and metabolically. When followed with attention to details we hear great success stories that include restored feelings of pouch tightness and satiation, lost weight, increased energy and overall improved confidence and mental well-being. 

 

If you are going to invest five days into getting back on track then do it all the way. Take the program as a whole and immerse yourself in doing it right. Build that storm of enthusiasm and recreate the excitement you felt leading into surgery and the days and weeks that followed. Learn about yourself. Take notes and use the 5DPT 2-page journal. Learn your strengths and weaknesses and harness that knowledge to do better on Day 6 and Beyond.

Question: I Didn't do WLS to be on a Diet! Why can't "the tool" just take care of my weight loss for me?

Recently a weight loss surgery post-op told me, "I did not have this surgery so I would have to monitor my calories and food. For years I gained weight while counting calories, carbs and such. I'm not about to do it now! I didn't have WLS just to be on another diet!"

I understand her feeling of frustration. Sometimes we want "the tool" to do all the work for us. But as we move further out from surgery we learn the doc's are not kidding about it being only a tool. To sustain a weight loss and achieve a healthy weight we must be mindful of nutrition, caloric intake and exercise output. This mindfulness is yet another tool for use in the pursuit of healthiness with weight loss surgery.


Question: My weight loss has stalled. Can I use the 5DPT to bust a plateau?

Answer:
If you follow the 5DPT you know I always say don't do the plan for quick weight loss. Do the plan to get back on track with our WLS basics and refresh your memory about the Four Rules, slider foods, and liquid restrictions. Any weight loss during the plan is a bonus. I stand by this.

But that doesn't mean the 5DPT should not be used to bust a weight loss plateau. In fact, many thousands of people have reported to me that the 5DPT is the one thing they tried that worked in breaking a weight loss stall.  It is not so much that the plan is magic, although who among us wouldn't like to believe in magic now and again! The plan breaks a stall and promotes weight loss because it returns us to the dietary due-diligence prescribed at the time of surgery and we focus on using the tools that make weight loss surgery work. Generally weight is lost in the first few days of the plan and this serves us well as a great motivator to continue with the plan and beyond knowing we are powerful and can work the tool to improve our health and manage our weight.

Question: I did the 5DPT and lost weight, but gained it right back when I went back to normal.  Why?

Answer: The motivation for doing the 5DPT should always be to get back on track with the WLS dietary guidelines, not to lose weight. That means we take what we learn during the 5 days and apply it to our lifestyle on Day 6 and beyond. If we do the 5DPT simply to knock-off a few pounds and then go back to the very habits that lead to weight regain we will, naturally, regain the weight and then some. When we consented to weight loss surgery we agreed that for the rest of our life we would follow certain dietary guidelines. If we have drifted from the original guidelines the 5DPT can get us back to basics. At that point we must follow the guidelines we agreed to if we wish to sustain weight loss and keep our obesity in remission. Use this as a means to return to following the instructions you were provided by your surgical weight loss center and nutritionist. Go back to doing what worked for you when you were at your best and losing weight or maintaining weight.

 
Question: Can I do the 5DPT every Monday-Friday 
and then take weekends off?
 
Answer: I hope you won't do this. It takes us back to our pre-surgery diet habits and serves no positive purpose. With surgery it is impossible to put-away the tool for the weekend, so why would we put away our good habits for the weekend? Use the 5 Day Pouch Test as it is designed and move forward with an eye on the long-term goal of better health and weight management making the most of your tool, each day, and every day.  

Question: What if the 5DPT shows me my
pouch is broken? What then?


Answer: If you have given the 5DPT your best effort and you still feel like your pouch is not working in the way it was intended to work please see your bariatric surgeon. Several diagnostic tests are available to determine the state of your pouch. In some cases a surgical revision may be necessary due to a pouch failure. Notice it is a pouch failure, not a personal failure when a revision is necessary. Revisions for gastric surgery are common and often patients are made to feel like moral failures if they require a revision. This is not the case. A revision is a medical procedure to correct a medical condition. Seek the care of a qualified bariatric team and do what is necessary to protect your health.



This is where I hope your find yourself after using the 5 Day Pouch Test   to get back on track. Take your knowledge and let it empower you because you are a good and worthy person. You are not your disease. I have met enough of you, my WLS Neighbors ,  to say this with complete conviction.

First: The 5 Day Pouch Test is not a plan to lose weight, although many people who do the test report weight loss. When we reach a point where we feel we need to "get back to basics" with our weight loss surgery the 5DPT is a tool to quickly get us back on track. Closely following the plan helps us break a slider food snacking habit that may have stalled weight loss or caused weight gain. We take our stomach pouch through the dietary progression we followed after surgery with two days of liquid pampering and three days of advancing from soft protein to firm protein to solid protein. During the 5 days we focus on the Four Rules, especially Protein First. We observe the liquid restrictions and focus on the very reasons we had weight loss surgery in the first place. Most importantly, the 5 Day Pouch Test gives us a renewed sense of confidence in our surgical pouch and our personal power in following a healthy eating plan that supports weight loss and weight maintenance. Any weight loss is incidental to that and should be considered a bonus, not the foremost objective.

Second: Seems like things have really heated up on the weight loss surgery scrimmage lines where it is more popular than ever to compare and compete with one another in our weight loss. And while some healthy competition can be a great motivator the comparison of weight loss can lead to utter discouragement. Frequently I see that people are making comparisons in how much weight is lost in the 5 days and if they do not meet that superficial standard they consider their 5DPT effort a failure.  It is not.

Unless you pound-for-pound and fork-for-fork competitively gained weight with another person you have no business competing to lose weight. Contrary to popular culture, weight loss is not a contest. Weight loss is a life saving initiative owned by the one taking action.

Can I repeat the liquid days instead of going to Day 3?

You can repeat the liquid eating plan of the 5DPT Days 1 and 2, but as soon as you do that you are doing a liquid diet; you are not doing the 5 Day Pouch Test. The intent of the 5DPT is to quickly progress through the post-op dietary stages and get us back to the basics of following our weight loss surgery high protein, low carbohydrate diet. People are sometimes tempted to repeat the liquid days because they have recorded a pleasing weight loss on Days 1 and 2. Keep in mind this weight loss is primarily water loss. In order to keep losing weight one needs to follow a high protein diet that elevates the metabolism into high thermic burn: this is when true weight is lost. Please follow the 5DPT as it is written: it was developed to help you achieve the best results with your weight loss surgery. We tested it numerous times so you don't have to. 5DPT: Days 1 & 2

Are dairy products okay to use on the 5DPT?

If you can tolerate milk and dairy products include them in your 5DPT and beyond. See the recipe section. Many WLS patients become lactose intolerant so that is why there is always a caution about using milk products. For those who can tolerate dairy, when used in moderation, it can be a healthy part of your diet.

Are yogurts or cottage cheese allowed on Days 1 & 2?

No, cottage cheese and yogurt should not be used as stand-alone menu choices on Days 1 and 2. However, they may be used as ingredients in protein smoothies. You can introduce yogurt and cottage cheese to your diet on Day 3: Soft Protein.
