Children's Medical Office Photo Contest!
We need your help! Would you like to be featured on our new website which is currently under construction?
We are looking for digital pictures of our patients 'dressed up' or playing pretend as healthcare providers, or our adolescent medical volunteers working in the field.
- RULES/GUIDELINES
- no identifiable patient info in the photo
- take pictures in a bright background
- doesn't need to be staged, have fun. cameos are better
- email the pictures to jacky@chmed.com with a subject line of "CMO Photo Contest"
- please include your contact information within the email
- ENTRIES SHOULD BE SUBMITTED BY July 1st
- We will contact you to obtain a signed consent if your photo is chosen
