Product Additions

We have updated our QHDHP benefit plans to have pharmacy copays after the deductible. This change was made to make our products more competitive in the market and to ensure the members see the cost share difference between generic and brand name prescriptions. The following products have been added to the product portfolio for new and renewing business effective March 1, 2013:
- Embedded QHDHP POS and PPO $3,000 Deductible, $4,000 Out-of-Pocket Maximum with Rx $3/$10/$30/$60/$60 or 20%
- Embedded QHDHP POS and PPO $5,000 Deductible, $6,000 Out-of-Pocket Maximum with Rx $3/$10/$30/$60/$60 or 20%
- SelectAdvantage POS $25/$500 20%
- Value POS $25/$50/20% $0 Deductible

Retiring Products
|
The following products will be retired in BenefitExpress effective March 1, 2013. Note: They still will remain available as options in the large group market but will be higher cost than the updated versions with pharmacy copays.
- Embedded QHDHP POS and PPO $3,000 Deductible, $3,000 Out-of-Pocket Maximum
- Embedded QHDHP POS and PPO $5,000 Deductible, $5,000 Out-of-Pocket Maximum
- Value POS $20/$40/20% $0 Deductible
The following products have been retired completely and are no longer available as standard products for small or large groups.
- CDHP PPO $25/$50/20% $1,500 Deductible
- CDHP PPO $25/$50/0% $5,000 Deductible
The SJ Products are a great alternative renewal for groups currently on the retired CDHP plans.

Product Changes

Effective March 1, 2013, the QHDHP suite of products will have pharmacy benefits similar to the ones offered on the SJ plans:
- Tier One - $10 (includes $3 for select generics)
- Tier Two - $30
- Tier Three - $60
- Tier Four - $60 or 20% up to $300 maximum per prescription
