To expedite this process, please be sure to include the Quote Request Form with every 2-99 submission. Not including this form with your quote(s) will delay the process.
Complete submissions will have a 48-72 hour turnaround. Missing information will cause a delay and it may take as long as 10 business days to turn these around.

What We Need From You:
- Quote Request Form ("Request for Medically Underwritten Proposal Form")
- Member Level Census - Using our template will allow for faster processing (template attached)
- Renewal - Complete current year on carrier paper (including employee detail)
- Current Invoice - Not needed if renewal includes listing of employees currently enrolled
