Clean submissions will have a 48-72 hour turnaround. Others may take a week to 10 business days.

What We Need From You:
- Quote Request Form ("Request for Medically Underwritten Proposal Form")
- Member Level Census - Using our template will allow for faster processing (template attached)
- Renewal - Complete current year on carrier paper (including employee detail)
- Current Invoice - Not needed if renewal includes listing of employees currently enrolled
