The HIV ACCESS PCMH Project Steering Committee held its first meeting of 2013 in mid-February.  Site representatives shared progress reports on PCMH-related work at their health centers (Please see"Intervention Update" for details.).

 

The Steering Committee also heard from Michael Horberg MD, Executive Director of Kaiser Permanente's Maryland-based Research Institute.  Dr. Horberg spoke about a potential study with the National Institutes of Health exploring optimal care teams for HIV care.  Several Steering Committee members expressed interest in participating.

Finally, the group received an update from the Project's Principal Investigator Kathleen Clanon MD, who is also the Medical Director for the Health Program of Alameda County (HealthPAC).  Dr. Clanon outlined the rapid changes taking place in health care at the federal, state, and local levels and how these changes will affect the Patient-Centered Medical Home movement.  Look for a video in our March newsletter!

Intervention Update 
  
At Alameda County Medical Center, a successful panel management intervention has been underway in the Adult Immunology Clinic since May 2012.  Clinic Manager Itta Aswad is currently preparing to hire a full-time Panel Manager.  This will enable the panel management pilot to be scaled up to all HIV providers at the Adult Immunology Clinic.
 
 At LifeLong Medical Care, providers are getting up to speed with the new NextGen Electronic Health Record.  The PCMH Project IT Systems Specialist is working with the EHR Project Manager to create functional "protocols" in NextGen that serve as decision support tools. 
 
At Tri-City Health Center, PCMH Coordinator Justin Weber is spearheading an effort to systematize the use of registry reports in the State Street Clinic.  Updated registries are used by both case managers and clinic RNs to track down patients who haven't been seen in 6 months or more and get them back into care.  Since the beginning of the year, the State Street team has brought 16 patients back into care using the registry reports.

One of the HIV ACCESS PCMH Demonstration Project's main communication strategies is using video to document our progress on our journey and to help us inform and engage our stakeholders. We're always on the lookout for other examples of using video in building PCMH momentum. See below for an example from Santa Rosa Community Health Centers, which highlights the potential impact of a patient-centered model. On February 26th and 27th, Project Coordinator Megan Crowley and a tean from Tri-City Health Center attended a Strategic Planning meeting for the HIV Medical Homes Resource Center (HIV MHRC). The meeting brought together Ryan White-funded agencies from four states to discuss how PCMH transformation fits into HIV primary care and Ryan White funding. The meeting featured a "Lessons from the Field" panel, workshops on PCMH concepts, and action planning sessions. 

 

Each clinic team conducted a SWOT (Strengths Weaknesses Opportunities Threats) analysis of their journey to PCMH transformation and recognition to help them better focus their efforts. Each team was also asked to develop an action plan for two specific content areas related to PCMH readiness (e.g. Quality Improvement Strategy, Engaged Leadership, etc.). Teams will expand and implement their action plans when they return home. The HIV MHRC will also be available to provide technical assistance to participating agencies. 

 

It was particularly helpful to learn from the experiences of other HIV providers in the Western U.S., particularly how they have leveraged different resources to support their transformation efforts. We look forward to hearing more about the HIV MHRC's future offerings and to seeing Tri-City's progress on their action plan! 

Evaluation Update 

  

Local evaluation team BTW informing change has completed their baseline data collection and will be presenting the preliminary findings to the PCMH Project Evaluation Advisory Committee next month.  Stay tuned for themes and takeaways from the patient focus groups, patient interviews, staff focus groups, and staff interviews BTW did at HIV ACCESS sites.  A full summary of evaluation activities to date will be included in our March newsletter.

Next Steering Committee Meeting: April 10, 2013

 

The next PCMH Demonstration Project Steering Committee meeting is scheduled for Wednesday, April 10th, 9:30am - 12pm at Alameda Health Consortium. More information will follow soon.
