Athletic Information

ATHLETICS -
PLEASE READ IMPORTANT UPDATED INFORMATION

 

Students planning to participate in athletics MUST have an athletic/eligibility packet on file with the Athletic Department PRIOR to the first day of practice/participation. The packet will be available during the summer in the Athletic Department in the Administration Building and at registration.   

 

The packet includes:

a) Completed Pre-participation Physical Evaluation Form/Physical Form (3 pages total)

**New this year -- must be dated after March 1, 2014 and must be signed by the doctor in two places**

b) Consent for Interscholastic Participation/Emergency Card - must be signed

c) AIA Form 15.7c Mild Traumatic Brain Injury (MTBI) / Concussion Annual Statement and Acknowledgement Form

d) Copy of birth certificate (First time participants only. If not provided student will be considered "practice only" until birth certificate is on file)

e) Copy of last semester report card (incoming 9th graders for fall sports only)

f) Acknowledgement of Rules and Terms for Interscholastic Participation Form - - must be signed

g) Certificate showing completion of Brainbook quiz from website: http://aiaacademy.org/students/register

h) Participation Fee per sport is $40. **New this year - The $40 fee will be collected after final rosters are determined.

 

Complete packets available at registration but please feel free to use above physical forms anytime.

Yearbooks are here and available for sale.

 

If you would like a yearbook and have not yet purchased one they are $65 and available in the MPR during first and second lunch.

Yearbooks can be purchased with cash or a check, the school is not able to accept credit cards or debit cards,  you can make arrangements to purchase a yearbook by e-mailing dalzner@amphi.com.
