Please note that due to a change in state requirements, everybody must provide proof of residence in the form of one of the following: a valid Arizona driver's license, motor vehicle registration, a current utility bill, or other acceptable proof listed on the Residency Documentation form.
Please be sure that any outstanding debts or unreturned textbooks are cleared through the bookstore. The bookstore has contacted parents earlier this summer regarding these issues.

The Public Health Department has 3 offices that offer school immunizations. They are as follows:
North Office - 3550 N. First Ave 243-2850
South Office - 175 W. Irvington 889-9543
East Office - 6920 E. Broadway 298-3888
All of these offices provide immunizations and the Public Health officials are encouraging parents to update their children now with required school immunizations instead of waiting in long lines in July and August. J
