Quadratus Lumborum Treatment
using Strain-Counterstrain
As you know, the Q L may be the cause of a nagging back ache. Patients may get up from sitting and feel like they can't straighten up for a few seconds before they finally achieve upright posture.
This short video gives a good look at how to release/diminish the tender or trigger points that are sometimes harbored in the Q L. It can be released using many different techniques, but here are my two favorites. By no means is this the only thing to do, as back pain is a multifaceted and sometimes quite complicated issue. Still, being able to calm a major back muscle down can be very helpful.

Q: Tom, I am writing to ask you a couple of questions. I have an adult patient with torticollis affecting the right SCM. It has been present for several months and has been getting worse. She has had injections including Botox which has not helped.
She was referred to us several weeks ago and we have begun treatment consisting of massage/myofascial release, ... MET stretching of the SCM, lower cervical mobilization of restrictions and HEP consisting of postural exercise and strengthening of the upper T-spine. She has been seen three times over the past ten days and has not noticed any change in her symptoms. Can you offer any further suggestions?

A: Hi Paul, My go to technique is counterstrain for the SCM. If you are working on the R counterstrain which means they are looking to the L. So in supine, take the head to full available L rotation and as much flexion (which actually looks like side bend at that point) until the muscles attachment at the SCJ is not tender. Hold for 90 seconds. It's tough on your wrist to hold for 90 seconds so rest the head partially on your chest.
If you have micro-current I would also use that at 30Hz for 10 min and .3 Hz for 10-20 min. Then work on active Rotation to the Right side to strengthen the likely inhibited L SCM.
