Gamers Players (and Parents):

It is time for the Spring Academic check-up for all players.

Please submit a copy of your latest report card and academic review form. Click here for background on the academic review process.



Please fill out the form, attach a copy of your most recent report card and send it in.  
 
Please do not turn in your report card without the form.  We need both!!
 
If you did not have a copy of your report card, then you can write in your grades on a separate page of paper.
 
Please send this to us by April 25th.   You can get it to us one of the 4 ways:

1- Drop it off at ASP

2- Scan or take a picture of it and email it to mark.gallion@gmail.com
 
3 - fax it to (636) 216-0086
