Medicare D Section Disclosure Requirement

The first of two MMA disclosure requirements was completed in September. The second disclosure requires employers to complete an Online Disclosure Form to CMS that specifies your health plans' creditable coverage status. More information regarding this CMS reporting, along with the link to the online form, is available here.


The Disclosure must be completed annually no later than 60 days from the beginning of the plan year.
