English and Spanish application forms are available at all Boise School District schools, the Food & Nutrition Services Office at the District Service Center or on-line at https://foodservice.boiseschools.org/

To apply for free or reduced price meals, households should complete one application per family through the on-line process or fill out an application and return it to the school or District Service Center.  The on-line process reduces the potential for errors on the application and therefore reduces processing time.  Applications may be submitted at any time during the school year. The information provided by the household is confidential and will be used for the purpose of determining eligibility for meal benefits.  This information may also be shared with education, health, and nutrition programs to help them evaluate, fund, or determine benefits for their programs, auditors for the program reviews, and law enforcement officials to help them look into violations of program rules.  The information provided may be verified at any time during the school year by District or other program officials.

Households must list the names of all household members, each household member's income and the source of the income on the application and include the name and last 4-digits of the Social Security number of the primary wage earner or the household member who signs the application. If the household member signing the application does not have a Social Security number, it must be indicated on the application. The signature of the adult household member who signs the application certifies that the information provided is correct.

Children receiving food stamps and foster children are directly certified to receive free meal benefits through the National School Lunch and Breakfast Programs. Households receiving food stamps or with foster children should contact the District Food Service Office if they are not receiving free meals.  Completion of an application should not be necessary. Households receiving Temporary Assistance for Families in Idaho (TAFI) or Food Distribution Program on Indian Reservations (FDPIR) must complete an application listing every child's name and their FDPIR or TAFI case number.  Foster children may be included on a household application with other non-foster children.  Foster children must be the legal responsibility of the welfare agency or the court.  Students who are homeless, runaway or migrant are also eligible for benefits.  Household members do not have to be US citizens for students to qualify for benefits.

Under the provisions of the free and reduced price policy, Boise School District Food & Nutrition Services officials will review the applications and determine eligibility. Parents or guardians dissatisfied with the ruling of eligibility may wish to discuss the decision with the determining official(s) on an informal basis. Parents wishing to make a formal appeal may make a request either orally or in writing to Peggy Bodnar, Food & Nutrition Services, 8169 W. Victory Rd., Boise, ID 83709 for a hearing on the decision.
