    Leprosy (Occupational Therapy)

     

    Mr. M is a 40-year-old patient being treated for wrist pain in the hospital's outpatient occupational therapy department. Mr. M has a history of leprosy.

    The occupational therapist wants to find some information on leprosy. She consults Rehabilitation Reference Center, keying in leprosy. She locates the clinical review "Leprosy (Occupational Therapy)."

    She learns a great deal about leprosy, including contraindications for occupational therapy, indications and pathogenesis. She then reviews the examination section of the clinical review. After completing the physical and subjective examination, she goes on to read about the treatment of patients who have leprosy.

    She advises Mr. M to follow his physician's recommendations for treatment of his condition.

    Functional Capacity Evaluation

     

    Functional capacity evaluation (FCE) is an umbrella term that refers to a set of tests and observations that evaluate a person's ability to perform work activities related to his or her participation in employment. FCEs are commonly used internationally for making fitness-for-work decisions. Many versions of FCEs are currently available and used clinically, with a wide variation in the items tested. FCEs are typically used after a workers' compensation claim to determine if an injury will inhibit a worker's ability to return to work. An FCE may be required by law for some employers before an employee can return to work and by insurers before insurance payments can be made. FCEs are also used to determine eligibility for disability insurance payments or pension eligibility when an employee is permanently unable to return to work. A job-specific FCE focuses on returning a worker to specific job tasks, whereas a generic FCE may be used for case closure. Validity, reliability and consistency vary depending on the specific tests and observations used.

    Evidence-Based Content Update
    Recently, the clinical review "Dysphagia: Moebius Syndrome" was revised following review as part of the Systematic Literature Surveillance Program. Information of value to speech therapy practice was found in a research study conducted in Brazil.

     

    In the study, researchers reported on oral motor function for a group consisting of 33 subjects (ages 2 to 20 years old) with Moebius Syndrome. Overall, there was great variability in the orofacial functions of the subjects in the study. Researchers found that 51.5% of the subjects exhibited adequate closing (i.e., functional range of motion) of the mandible; however, strength was reduced as compared to a control group without Moebius Syndrome. In 7 of the subjects, there was no voluntary mandibular movement present. With regard to labial closure, 39.4% of the subjects were completely unable to bring the lips to a closed position
