Documentation Corner: DNR
Definition
In medicine, a "do not resuscitate," or "DNR," sometimes called a "No Code", is a legal order written in the patient's medical record to respect the wishes of a patient not to undergo CPR or advanced cardiac life support (ACLS) if their heart were to stop.
Procedure
At Medical University Hospital Authority, in an effort to use language that will be better understood by families, documentation of "Allow Natural Death" is approved as synonymous to DNR and will be coded a such. Medical record documentation for DNR/Allow Natural Death may be located but not limited to the death progress note, H&P and Advanced Directive.

Head's Up!
Great Southeast Shakeout
At 10:17a.m. on October 17, 2013, all of MUSC will practice "Drop, Cover, and Hold On" for The Great SouthEast ShakeOut.
The earthquake drill: everyone get under a table or desk and hold on for 10-20 seconds. Participants should hold on to unsecured tables and desks because during an earthquake they will MOVE!
Prizes will be given for the best earthquake drill picture submitted to Amanda Ritsema by a department, laboratory, and classroom setting.

BOT Presentation & Key Messages |
Dear MUSC Medical Staff,
This week the MUSC Board of Trustees are in town. I would like to share with you a few of the presentations made by senior leaders. The following is my presentation, which focuses on:
- Current goal performance
- MUSC Performance Excellence, which is our financial improvement plan
- MUSC Health Plan
We have made much progress in the past 6 months in terms of our financial improvement plan, as well as the development of the MUSC Health Plan. None of this is possible without significant engagement of all MUSC employees and medical staff!

On September 26th, we had a Physician Leader meeting and as I have stated in previous Clinical Connections, the medical center communications team develops a key message presentation for those leaders to share with you. However, we realize because of the timing of department meetings that this is not always possible and it is my goal to share these messages directly with you 2 weeks after the Physician Leader meeting. Discharge Planning Standard Time
Persistent delays in our discharge process have created bed shortage and bottlenecks for patient admissions. Additionally, MUSC remains out of phase with comparable hospitals in UHC. To improve discharge for our patients, we have instituted several changes:
MUSC Medical Executive Committee endorsed standard metric Discharge Times
-Discharge order should be on patient's chart by 10am the day of discharge
-Patient should be discharged by 1pm (i.e. 3 hours after order)
Discharge Team Rounds
The medical center has also increased the frequency of discharge team rounding to twice a day.
The schedule for discharge team rounds is as follows:
Adult Hospital
10:00 AM - Main Hospital and ART
3:30PM - Main Hospital and ART
Pediatrics
8:30AM - Children's Hospital
3:45PM - Children's Hospital
The culture of discharge team rounds has evolved; we now focus on identifying patients for discharged, as well as problem-solving and process steps to ensure each patient is ready for discharge.
