ICU stays may have long term effects on cognition. In the New England Journal of Medicine, researchers studied survivors of critical illness in the Bringing to Light the Risk Factors and Incidence of Neuropsychological Dysfunction in ICU Survivors (BRAIN-ICU), a multicenter prospective cohort study. Patients who had a substantial ICU stay were assessed for duration of delirium and use of sedative or analgesic medications. 74% of patients experienced delirium; 40% of these patients had cognition scores worse than those seen in traumatic brain injury (TBI) at 3 months. At 12 months, 34% of patients had scores similar to TBI. Benzodiazapines were associated with lower cognitive scores at follow up, but other sedative medications (including propofol and desmedtomidine) did not.
