Hidden Danger

 

When most people think about simulation in healthcare, they think about learning skills that can be taken back to the bedside. However, another benefit of simulation training is the identification of potential safety threats. This can be done during "in situ" simulation whereby team training is conducted in the patient care area. During this exercise, hidden safety threats may be discovered. A team at Cincinnati Children's Hospital performed 90 in situ simulations and discovered 72 latent safety threats. Examples included missing or broken equipment, lack of familiarity with resource materials and environmental problems such as restricted views of the monitors to members of the team. The authors note that performing clinical simulations with working clinical teams presented logistical challenges, including the distraction of a busy ED. However, the realistic teamwork training and latent threat identification made the effort worthwhile.
