Patient and Family Engagement

The National Patient Safety Foundation recently posted on their website a list of tools identified as best practice for engaging patients and families in their care. This 49 page document was compiled by the federal Partnership for Patients (PfP) program by surveying hospital engagement networks, PfP stakeholders, PfP hospital executives, and patient advocates. Tools are divided into three categories: clinical/point of care; policy and design; and governance. Each tool is presented with a description of its purpose and a link to the resource. Topics include error and readmission prevention, improving communication between patients and clinicians, and information for vulnerable patient populations.
