Reducing Mortality |

The University of Pennsylvania Health System in Philadelphia was able to decrease their mortality index as measured by the University HealthSystem Consortium (UHC) from 1.08 to 0.53 through a focused effort by a hospital-wide Mortality Review Committee. In 2006, questions regarding the validity of administrative data were addressed by medical records working with the responsible caregivers of recently deceased patients to add addendums to improve the accuracy of diagnoses and procedures. All clinical staff that cared for the patient were asked if there were any problems with the patient's care that needed to be addressed systemically. The Mortality committee used this data to develop system wide quality improvement initiatives, including ones that addressed sepsis and delirium. Keys to success included multidisciplinary involvement and leadership support.
