Present on Admission (POA)

At the end of a patient's hospital stay, the medical chart is thoroughly dissected by our coding department, to determine what procedure and diagnosis codes are appropriate.

 

If a diagnosis is not clearly described in the chart as being "present on admission", it is coded as occurring during the hospital stay. For some conditions, this results in a "hospital acquired condition" or HAC, which can affect our quality rankings and our reimbursement.

 

The most common conditions not clearly documented as "present on admission" are pressure ulcers and urinary tract infections. Please be sure to document when a condition is present on admission, to avoid unnecessary HACs.

New Patient Satisfaction Survey

Dear MUSC Medical Staff,Dr. Cawley

  

As of July 1, we now have a new ambulatory patient satisfaction survey. The Clinician and Group Consumer Assessment of Healthcare Providers and Systems survey (CG-CAHPS) is now in all adult MUSC Health clinics. The pediatric version of this survey is not yet available and the pediatric clinics will continue the current Press Ganey survey.

 

This is a different survey than previous Press Ganey and Avatar ambulatory surveys. The CG-CAHPS survey is designed to measure the physician office patient experience. CG-CAHPS asks patients to report on their experiences with providers and office staff at their most recent visit to a doctor's office. View the full survey.

 

Why change?

  1. Compare - This survey is part of a growing policy to capture the patient experience in a standardized manner that can be compared across health organization. Results will be publically reported.
     
  2.  Payment - The survey is not yet mandated by the Centers for Medicare and Medicaid Services (CMS), but CMS has begun linking certain payments to performance on this survey. It is expected that all providers who accept Medicare and Medicaid in the future will be required to capture the patient experience via CG-CAHPS.

To help you learn more about this new survey, we are sponsoring a seminar led by a physician, Dr. Jeff Morris. He will explain the new survey and tell us the best ways to approach patients. Dr. Morris is an orthopedic surgeon with a special interest in hand surgery. He is a Fellow of the American College of Surgeons and board certified in orthopedic surgery through the Royal College of Physicians & Surgeons, Canada

Things You Should Know About

MUSC's Pharmacy Department

  1. Retail & Mail Order - We offer 4 on-campus retail pharmacies and an off-site mail-order pharmacy that provide care to patients, employees and physician staff. We would appreciate your help in sending your patients to our pharmacies.
     
  2. High Level Review - We offer specialized services, high-level review of prescriptions and customer service that is unparalleled out in the community.
     
  3. Discharge Medication Counseling - We have a discharge medications counseling and delivery service for discharging patients. We need your help identifying discharging patients EARLY so we can get to them before they go home. 
     
  4. Patient Rounding - We have clinical pharmacists that round daily. Medically intensive services require pharmacists with specific expertise for their patient populations.
     
  5. Anticoagulation Management - We offer inpatient and outpatient anticoagulation management. 
     
  6. Pharmacokinetic Monitoring - We have pharmacists who provide pharmacokinetic monitoring.
     
  7. Patient Education - We offer patient education to all patients in the hospital.
     
  8. Drug Shortage Management - We send weekly drug shortage email updates, which are distributed by the Chief Medical Officer's staff. These emails will help you stay aware of the quickly changing (and very frustrating) nature of important drug shortages.
     
  9. Interdisciplinary Approach - We have interdisciplinary committees with the input and guidance of physicians, nurses and other colleagues that guide medication use process at MUSC. The major reporting committee is the Pharmacy and Therapeutics Committee, chaired by Dr. Larry Field. We encourage your open communication with our teams to help you successfully practice medicine at MUSC. 
     
  10. Pharmacy Residency Program - We have one of the largest and most diverse pharmacy residency programs in the country with specialty residents involved with many services across the hospital.
