Patient and Family Communication

Effective communication between caregivers, patients and families promote patient safety, however, communication problems are common. The Agency for Healthcare Research and Quality (AHRQ) has published a guide that provides four evidence based strategies to improve patient and family engagement and effective communication. Those strategies are:

  • Promote patient advisors-patients and families should be recruited at the organizational level as advisors to the hospital as partners, working with hospital based committees and leaders.
  • Education for patients and families at the time of admission to promote partnering in their care, including information about the health care team.
  • Encourage patients and families to participate in nursing bedside report.
  • Robust discharge planning that includes ongoing planning throughout stay and use of checklists
