Handoffs-What Works? |

Most clinicians will report that handoffs, the communication of clinical information from one practitioner to the next, is a very problematic practice. Handoffs and care transitions have been among the lowest scoring results in MUSC's survey on the Culture of Safety for years, but little data exists to determine what the best course of action in correcting this problem. A systematic review of handoff tools was published this month in the Journal of the American Medical Informatics Association; the authors find that there is large variation in the method of evaluation and the quality and rigor of the studies. As with many other subjects, further research is needed.
