Hospitals, patient safety organizations and insurers in Massachusetts have come together to create a new way of handling adverse events in health care systems. Based on previous work done in Michigan and Stanford, the Massachusetts Alliance for Communication and Resolution following Medical Injury (MACRMI) endorses the CARe approach, which emphasizes disclosure to patients and families; investigation and explanation to them regarding the cause of the event; implementation of system changes to avoid reoccurrence and, when appropriate, apology and offer of financial settlement without lawsuit. Implementation of a similar system was shown to decease liability costs by a third. It should be noted that the Massachusetts group received support from their state legislature who created enabling legislation for the effort.
