Reporting Residents |

A reporting culture, whereby front line staff report safety concerns to leadership, is an important part of a safety culture and is a characteristic of a highly reliable organization. A "rich" source of safety information in academic medical centers are house staff. Unfortunately, most do not regularly utilize reporting systems. A study in the Journal of Patient Safety examined the effects of an educational program and a reporting expectation from the Graduate Medical Education (GME) office; they found a sharp increase in reporting immediately after the program was initiated; however, the improvements were not sustained.
