Improving handoffs and transitions between caregivers is a patient safety priority, yet a difficult problem to solve. One factor in this difficulty is the variability of different practitioners, who need to communicate differently, e.g. a nurse calling report from the ED needs to communicate different information than does a resident performing an end of the day handoff for a ward service. There is also variability within practitioner groups. Nursing shift report was studied by a group from MIT, and extensive variability was found; incoming nurses wanted to ask questions; outgoing nurses wanted report without interruption. Novice and experienced nurses also had different expectations. Although standardization of handoff communication should improve communication, it may be difficult to achieve.
